SUMMARY
OF KEY EVENTS Week Ending June 8,1979 Arkansas Nuclear.One - Unit 1 On June 2 during the operation of starting Arkansas Unit 1, Arkansas Power and Light Comoany was issued an Order by NRC requiring the plant to return to cold shutdown condition. The Order resulted from the resident inspector finding the Emergency Feedwater System locked out of automatic initiation mode. The plant will remain shut down until the licensee has satisfied certain conditions. Brunswick Units 1 and 2 Due to discovery of underdesigned pipe supports and hangers, Carolina Power and Light Company decided to shut down both units. CP&L will investigate and make all nncessary modifications to supports before beginning return to power. On June 7, NRC perfomed a safety evaluation concurring in the return to power operation on the basis of completed modifications and satisfactory results of pipe stress reanalyses. Oyster Creek On May 30, Jersey Central Power and Light Company was authorized to resume operation of the Oyster Creek plant. The plant had been shut down since May 2 when a loss of feedwater transient occurred. San Onofre U, nit 1 The facility was shut down on June 2 for repair of a tube leak in steam generator A. During this shutdown, the licensee is examining feedwater line connections to the nozzle of steam generators A, B and C. PWR Feedwater Line Cracks As a result of feedwater line cracks found at the D.C. Cook facilities, a letter was issued on May 25 to all PWR licensees requesting information on the design, fabrication history, inspection, and operating history of these lines. Hearing boards and the Division of Systems Safety are being notified of preliminary information for_ consideration as a_ potential generic problem. Non-croliferation Language in NRC International Aoreements IP was adhised by letter June 1 that the Executive Branch recentnends the adoption of cotion 2, as detailed in SECY-79-95. An action paper has been prepared and is en route to the Commission. Emergency Precaredness Radiological Emergency Response Plans from Virginia, Nebraska and Colorado have been pre::ared for final review by the appropriate Federal Regional Advisory Committees. WEEKLY INFORMATION REPORT A commission paper forwarding the NRC Systematic Review Guidelines, for use by the Archivist of the United States in declassifying 20 year old National Security Infomation under NRC jurisdiction (i.e., originated by AEC regulatory program), has been prepared, and staffed. When approved, the Chaiman will furnish the guidelines to the Archivist in accordance with the provisions of Executive Order 1206s. " National Security Information." On June 4,1979, a security survey was conducted on the Resident Fuel Cycle Inspector at Nuclear Fuel Services, Inc. (NFS), Erwin, Tn. to per-mit him to receive, store and safeguard classified matter. On June 6,1979, a representative of the Division of Security and three employees of the Department of Commerce, National Telecommunications and Informati'an Administration (NTIA) visited the U.S. Tennessac Valley Authority Watts Bar Nuclear Plant in relation to the upcoming NTIA survey of NRC. The Division of Security initiated the fomal security survey of the Headquarters offices on June 4,1979. A briefing of designated NRC representation on the role and purpose of Department of Commerce, Natirnal Telecommunications and Information Ad-ministration was conducted by NTIA personnel on June 1,1979 in prepa-ration for their upccming survey of NRC. On June 5,1979, Facilities and Systems Security Branch personnel coordinated guard coverage at 1717 H. St. in response to a demonstration conducted by Friends of the Philipino People in their campaign for a nuclear free Philippines. On May 31,1979, the Facilities and Systems Security Branch conducted an initial security survey of the Battelle Memorial Institute located in Columbus, Ohio with satisfactory results. CFFICE CF NUCLEAR REACTOR REGULATION MEEKLY ITEMS OF INTEREST (Week Ending June 8, 19/9) Arkansas Nuclear One - Unit 1 Arkansas Power and Light Company was given permission to resume operation after having completed the short-term requirements of the Order of May 17, 1979 which required the plant to remain shutdown. The plant had been shutdown since March 30, 1979 when it shut down for a refueling. The amendment which revised the Technical Specifications to reflect plant operating limits for-the fuel loading for Cycle 4 operation was issued May 23, 1979.

0FFICE OF STANDARDS DEVELOPMENT IMPORTA::T EVENTS FOR THE WEEK ENDING JUNE 8,1979 1. EPA /NRC Meeting on Clean Air Act: On June 5,1979, members of the ~ NRC/ EPA technical and legal staffs met at EPA for further discussion on the Clean Air Act. Three topics were discussed: (1) the NRC regulatory program for ALARA and 40 CFR 190 implementation for fuel cycle facilities, (2) the various categories and numbers of non-fuel cycle licensees and potential doses to the public, and (3) the NRC/ EPA interagency agreement required by the Act. The EPA representatives d;;cribed what additional technical information they need from NRC to pursue the use of 40 CrR 190 as a Clean Air standard and to plan development of standards for non-fuel cycle facilities. We agreed to provide the additionai information. It was also agreed that the EPA and NRC staffs would develop separate preliminary outlines of an interagency agreement within two weeks and then meet to develop a joint outline. [J. Hickey] 2. Human Uses of Bvoroduct ?ial: On June 7,1979 proposed r.mendments to Parts 30 and 35 were published in the Federal Register for 60-day public comment. These proposed amendments would require medical licensees, who use radioisoto,ne generators, to test technetium-99m radiopharmaceuticals for the contaminant molybdenum-99. OFFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest Week Endina June 8, 1979 Export of HEU to Canada A portion (87.752 Kgs U-235) of the HEU authorized for export under Export License XSNM-1292, was shipped from the Union Carbide plant at Oak Ridge, Tennessee, (departing at 8:35 a.m., June 1,1979) to Chalk River, Ontario, (arriving at about 9:30 a.m., June 3,1979) without incident. The shipment das made in a special cargo vehicle with bullet-resistant tractor and compar runtalized trailer and escorted by two vans. Seven armed guards crewed the canvoy to the Canadian border where Ontario Provincial Police assumed security responsibility and the two armed drivers of the cargo vehicle disarmed in accordance with Canadian law. There were no security incidents during the shipment. The initial departure was delayed several hours because of vehicle maintenance problems prior to arrival at Oak Ridge. At one point it was necessary to stop and perform other maintenance. During this and other stops, security in accordance with 10 CFR Part 73 and license conditions was provided. This is the shipment that was scheduled for two moves of approximately 45 Kgs. each; hnwever, NMSS/SG required that it be consolidated into one, based on the premise that splitting the shipment was contrary to good safeguards practice and that ccasolidation was preferred. Oconee-McGuire Testimony Safeguards testimony concerning tae risk posed by the shipment of spent fuel from the Oconee reactor in South Carolina to the McGuire reactor in North Carolina has been forwarded to ELD for use in the related public hearing pro-ceedings. Physical Protection of Soent Fuel Shioments A new guidance document, NUREG 0561, is F ,g arepared to support the new physical protection amendments for spe r el r rotection. Items of Interest 2 GA0 Report on Transportation The final version of the GA0 report entitled " Federal Actions are Needed to Improve Safety and Security of Nuclear Shipments" has been issued. We are preparing for the Chairman's signature a statement of actions taken on the GA0 recommendations to be forwarded to the House and Senate Committee on Government Affairs. Transient Shioment Rule Office comments on the draft Transient Shipment Commission Paper, proposed rule, and draf t guidance have been received and are being incorporated into the final paper. The entire package will be forwarded for office concurrence the week of June 11-15. Meetings held with the United Nuclear Corporation Members of the Division of Safeguards met with representatives of United Nuclear Corporation Recovery Systems on June 5. Material control and accounting for the Lagoon Recovery System presently under construction, was the major topic discussed. On June 6, a second meeting was conducted with personnel of UNC Recovery Systems and UNC Naval Products Division. The discussion in this meeting involved shipper / receiver differences between the two UNC fdcilities. 2 NBS UFg Cylinder Artifact Procram Representatives from the National Bureau of Standards met with members of the Safeguards staff on June 4 to discuss their UF6 cylinder artifact program. On June 7, Mr. C. W. Emeigh of the Safeguards licensing staff attended an INMM sponsored educational meeting in Atlanta where instruction was given on how to use the NBS UF6 cylinder artifacts. The purpose of the meeting was to create in-house UF6 cylinder standards at those fuel facilities which handle these cylinders. m ENCLOSURE D bbb l.? Q
OFFICE OF INSPECTION AND ENFORCEMENT Items of Interest Week Ending June 8, 1979 1. Arkansas Nuclear One, Unit 1 - Order to Return to Cold Shutdown - On June 2, 1979 while the plant was in a hot shutdown condition, an NRC inspector discovered that the Emergency Feedwater System was locked out of the automatic initiation mode. A procedure for the surveillance test in progress did not specify locking out the emergency feedwater auto initiation capability. Lacking a procedural requirement to return the system to normal, there was no assurance that emergency feedwater would be provided automatically if needed. An order was issued on June 2, 1979, which required the licensee to return the plant to a cold shutdown con-dition until the licensee had satisified certain conditions. 2. The following Notification of Significant Enforcement Action was dispatched during the past week: a. Eth79-04 Niagara Mohawk Power Corporation (Nine Mile Point) - Proposed Impositicn of Civil Penalties - $18,000 - On June 5,1979, a Notice of Violation and Notice of Proposed Imposition of Civil Penalties was issued to the subject licensee. This action is based on six alleged items of noncompliance with NRC requirements, all of which deal with site security. 3. Preliminary Notifications relating to the following actions were dispatched during the past week: a. PNO-79-67AR Three Mile Island Unit 2 - Nuclear Incident at Three Mile Island - This Preliminary Notification was issued to provide updated status information. b. PNO-79-131 A North ' Anna and Surry Power Stations - Operator Strike - The licensee offered a revised contract to the International Brotherhood of Electrical Workers (IBEW) on June 2. It appears that work will con-tinue until at least June 25, when a vote on the contract is scheduled. (Closed) c. PNO-79-132A Turkey Point Units 3 & 4 - Diesel Generators Inoperable - The licensee has identified and corrected the source of the transformer failures. Both diesel generators passed a 24-hour full load test and have been declared operable. (Closed) d. Several water samples collected by students from Brown University and analyzed by a private laboratory indicated concentrations of radioactive materials several hundred times higher than the NRC samples collected in October and November 1978. Region I (Philadelphia) has arranged with the State of Massachusetts to have additional water samples taken at the Norton site. There samples will be analyzed by both the Idaho Radiological and Environ-mental Services Lab and Interex, the private laboratory utilized by the Brown University students. Subsequent actions will depend on the rciults of the analyses. (Closed) f. Robinson Unit 2 - Reactor Coolant Pipe Lagging Fire On June 3,1979, during plant heatup, a, fire was detected in the pipe insulation on the reactor coolant piping in the containment building. The fire was quickly extinguished with dry chemical. The fire was caused by reactor coolant pump motor oil which had spilled on the reactor coolant pipe insulation during the refueling outage. At the time the oil ignited, the reactor coolant temperature was 520 degrees F, approximately 100 degrees above the oil flash peint. Subsequer.t to the fire; the plant was cooled to 380 degrees F in order to remove insulation, inspect piping and install new insulation. (Closed) h. PNO-79-137 San Onofre Unit 1 - Crack Indications in Steam Generator Tubes - Eddy current examinations on the tube in the "A" Steam Generator showed that 17 out of 670~ tubes tested have crack indications ard will require plugging. Because of the extent of tube damage found, eddy current tests will be conducted on a campling of tubes in the "C" Steam Generator. Dependent upon these results, a decision will be made whether to inspect the "B" Steam Generator. (Closed) 1. PNO-79-138 San Onofre Unit 1 - Crack in Feedwater to Steam Generator Held - As a result of a' letter sent to all PWR licensees from the Division of Operating Reactors /NRR, radiographic examinations were performed on the "B" feedwater to steam generator nozzle welds. The results show a cir-cumferential crack in the "B" feedwater nozzle to the pipe reducer weld. Ultrasonic tests are being conducted to further evaluate the radiographic incications. The "A" and "C" feedwater to steam generator nozzles will also be examined. The current maintenance outage is expected to be extended for an undetermined period. (Closed) j. PHO-79-139 Hatch Unit 2 - Failure of Backup Core Cooling Systems - On June 3, one of two ocerating feedwater pumps tripped causing a low water level reactor trip. The Reactor Core Isolation Cooling (RCIC) turbine started autcmatically as repuired but isolated due to a ruptured exhaust diaphragm. The licensee's investigation revealed a problem in the check valve en the turbine exhaust line, apparently causing a temporary flow blockage. Replacenent valve parts have been ordered and the RCIC will be repaired prior to startup. The High Pressure Coolant Injection (HPCI) mmp was signalled to start automatically during this event but did not start because the steam supply valve to HPCI turbine did not open. The ENCLOSURE E . -. ~ _. \\il y' failure to open has been attributed to water in the oil control system. This problem was traced to a closed valve in the HPCI turbine cavity drain. The valve has been locked open. Unit 1 does not have this valve, but generic implications are being pursued. (Closed) k. PNO-79-140 International Spent Fuel Shipment - NRC Region V to Provide Radiation Survey Assistance to U. S. Coast Guard, Honolulu, Hawaii - A ship enroute to England with a shipment of spent reactor fuel from a Japanese reactor is scheduled to arrive in Monolulu on June 8,1979, for a refueling stop. Because of high public laterest, the U. S. Coast Guard has requested NRC assistance in performing a radiation survey of the spent fuel cargo upon arrival in Honolulu. A Region V (San Francisco) radiation specialist will conduct the requested survey. (Closed) 1. PNS-79-56 Shearon Harris Units 1, 2, 3 & 4 - Bomb Threat - The plant site was evacuated and a search conducted with negative results (Closed) m. PNS-79-58 Nuclear Fuel Services, Erwin, TN - Inventory Results - The inventory difference for the highly enriched uranium operations for the period ending May 7, 1979 exceeded its associated limit of error, but did not exceed the limits of the license condition requiring reinventory. The licensee has initiated an investigation as required by 10 CFR 70.53(b). Regional inspectors are onsite assisting the resident inspector in con-ducting independent measurements as part of the intensified inspection program at NFS. The plant is currently in a shutdown condition due to an operators strike. (Closed) 4. While the Licensing Board conceded that the Indian Petitioners possessed the requisite " interest" and had specified their " contentions" to meet the requirements of 10 CFR 52.714, the Board concluded, after weighing the factors specified in 10 CFR SS2.714(a)(1) and (d), that the balancing factors of the evidence was against entertaining the petition and that it should be denied. He met with IP, NRR, SD and RES staff members to discuss upcoming safety coarses he is managing for the IAEA. Discussions cen-tered on identifying ways of replacing, as lecturers, NRC staff who are tied up on TMI matters, for two courses scheduled this fall (a safety analysis course to be held in Korea and a siting course to be held at Argonne). He also discussed the timing and the development of a course at Argonne on sa'fety reliability in nuclear plant operation to be held sometime.in the next twelve months. Nonoroliferation Lancuaae in NRC International Acreements IP was advised by letter dated June 1,1979 that the Executive Branch recomrends the cdoption of option 2, as detailed in SECY-79-95. An action paper has been prepared and is en route to the Commission. TMI-Related Information IP is preparing a summary of the reports and actions taken by foreign countries in the wake of TMI-2. Several of the countries which sent delegations to NRC in the early days of the incident have now provided us with their preliminary assessments and recommendations for their own nuclear power programs. ~. This notice concerns tritium contamination of food at a food assembly plant located near a large critium processor in Tucson, Arizona. The tritium processor is an Arizona licensee. Emercencv Preparedness Radiological Emergency Responce Plans from Virginia, Nebraska and colorado have been prepared for final review by the appropriate Federal Regional Advisory Committees.
