NPPD Response to IE Inspectice Report 50-298/79-10

Dear Mr. Seyfrit:

This letter is written in response to the letter dated June 6, 1979 transmitting IE Inspection Report 50-298/79-10 which indicated that l

certain of our activities were not conducted in full compliance with NRC I

requirements.

Statement of Infraction Criterion VI of Appendix B to 10 CFR 50, requires that measures shall be established to control the issuance of documents, such as instructions, procedures and drawings, incluJing changes thereto, which prescribe all j

activities affecting quality.

These measures shall assure that docu-ments, including changes, are reviewed for adequacy and approved for release by authorized personnel and are tilstributed to and used at the location where the prescribed activity is performed.

I The Cooper Station QA Manual, Section 2.6, requires that procedures, including revisions, for activities affecting quality be controlled and

distributed to the locations where the activity is taking place.

Contrary to the above requirements, on May 1, 1979, maintenance was being performed on a standby liquid control system relief valve, an activity affecting quality, and the latest revision (Revision 2) of controlling procedure, Procedure No. 7.2.35, was not at the work lo-l cation.

Corrective Steps Which Have Been Takan and the Results Achieved We have made a thorough revies of this event in an effort to determine the cause of the latest revision of the procedure not being used. We found that the controlled copy file cf the subject procedure 7.2.35 did not contain any copies of the latest revision.

The persaanel involved used a copy found in a desk which appeared to be the same.

However, the procedure revision number was not checked.

Some references had been added to the latest revision for convenience purposes.

The personr.el involved have been instructed to use the latest revision of procedure from the controlled copy file.

Corrective Steps Which Will Be Taken to Avoid Further Noncompliance We will audit use of procedures to deternfue if this was an infrequent event or a common problem. We have changed our method of distribution of revised procedures to the controlled copy file location.
