Item Unit 1 ANO procedure 1005.18, revision 0, Control of Ign_ition Sources, has been established and requires in paragraph 3.3.2 that the cognizant craft supervisor shall fill our an ignition source permit for work involving ignition sources such as welding, cutting, grinding or open flame work.

Contrary to the above, the inspector cbserved work being done in the Engineered Safety Features A3 and A4 switchgear room cn March 21 and 22, 1979, involving welding and grinding (reference Bechtel Work Request 1-78-60, dated November 13, 1978, and DCR 589) and for which no Ignition Source Permit has been issued.

Response

Control of Ignition Sources Procedure 1005.18 is being revised to redefine the term " ignition source", to clarify the term " cognizant supervisor", and to correct some other minor errors.

The expanded definition of ignition source includes all spark and flame producing sources. Work areas having exposure to flammable liquids, combustible materials or explosive atmospheres are required to be inspected and appropriate fire-explosive-accident prevention measures taken.

A note was added to the procedure to clarify " cognizant super' visor".

The cognizant supervisor is that "AP&L or contractor supervisor who is directing or supervising the activity that causes the need for that ignition source permit."

Distribution of Procedure 1005.18, Revision 1, " Control of Ignition Sources" is expected to be completed by June 5,1979. The plant manager will write a memo to the supervisors instructing them to review procedure 1005.18 with their personnel. The review will be completed by June 30, 1979. Quality Assurance will monitor the effectiveness of the revised procedure.

. . m Unit 1 and 2 M10 procedure 1005.17, Control of Combustibles, has been established and section 4.3.3 of this procedure requires that the work sites in safety related areas be inspected and that all unnecessary debris, waste, scrap, rags, and other combustibles be removed each 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> and at the end of the job.

Contrary to the above, two safety-related areas in Unit 1 and two safety-related areas in Unit 2, had accumulations of debris from completed work activities that were not removed as required.

Response

The Plant General Manager issued a memorandum to all supervisory personnel informing them of this non-compliance and instructing the cognizant supervisor of the men performing the work to inspect the work area at least once per day to insure that all unnecessary waste, debris, scrap, rags, oil spills or other combustibles resulting from the work activity are removed each day.

This memorandum is also being reviewed with all operations, maintenance, and appropriate contractor personnel. This review shall be completed by June 5, 1979, at which time we will be in full compliance.
