Appendix A NOTICE OF VIOLATION Midwest' Inspection License No. 48-16296-01 Services, Ltd.
Based on the inspection conducted on May 24, 1979, it appears that certain of your activities were in noncompliance with NRC require-ments, as noted below. These items are infractions.
1.
10 CFR 20.101(a) limits the whole body exposure of an individua) in a restricted area to one and one quarter rems per calendar quarter, except as provided by 10 CFR 20.101(b).
Paragraph (b) allows a whole body exposure of three rems per calendar quarter provided certain specified conditions are met.
Contrary to this requirement, one individual working in your restricted area received a whole body radiation dose of 1.340 rem during the third calendar quarter of 1978 and 2.150 rem during the fourth calendar quarter of 1978, and the conditions of paragraph (b) were not met.
2.
10 CFR 20.405(a) requires that you submit within 30 days, a report to the Commission concerning each exposure to radiation in excess of any applicable limit in Part 20 or in your license.
Contrary to this requirement, as of May 24, 1979, you failed to report to the Commission a 1.340 rem exposure received by an individual during the third calendar quarter of 1978.
