Koester letter to Seidle dated 4/25/79 Sub: Potential 50.55(e) Report Regarding Welds on Gr.W Pipe Assemblies

Dear Mr. Seidle:

This letter is to provide additional information regarding the Gulf

& W stern preassembly welds discussed in ny letter to you dated Ispril 25, 1979 (Reference (b)).

The observed condition of welds I:o. 1 and 8 of preassembly A-9-111 were documented and a:_spositioned on IIonconfornance Report tio. IS!;0863M.

The disposition provided for grinding the OD of the welds, smooth cutting the elbows in half and grinding the ID of the elbows to move a counter bore ridga away from tho

1d root, being careful not to grind actual weld metal.

The welds were then re-radiographed.

The radiographs showed that these welds did not have lack of fusion. What had been suspected of being lack of fusion was actually the interior diameter counter bore ridge. However, because the elbows were destroyed by this testing, diey will be replaced.

Because of questions raised regarding preassembly A-9-lll, an extensive review of radiographs for other preassemblies was conducted. This re-view established that a substantial number of the radiographs do not meet the requirenents for density, penetrameter placement, fi' m identi-fication, etc.

D1 ring a June 12, 1979 meeting at the Vendor's office, an action plan was agreed upon to correct there radiograph quality problems.

The program includes visual inspection of all Q-listed pre-

Should these further investigations indicate weld defects or other serious deficiencies, such de ficiencies will be evaluated for report-ability to the NRC pursuant to 10 CFR 50.55(e).

In the interim, no further action or reports appear necessary.
