APPENDIX A NOTICE OF VIOLATION Grinnell College License No. 14-10271-01 License 50. 14-10271-02 Based on the inspection conducted on May 22 and 23, 1979, it appears that certain of your ectivities were in noncompliance with NRC require-ments, as noted balow.
Items 1 and 3 are infractions, and item 2 is a deficiency.
License No. 14-10271-01 1.
License Condition No. 13 of Byproduct :hterial License No. 14-10271-01 states, in part, the licensee shall possess and use byproduct material in accordance with state =ents representations, and procedures contained in letter dated January 15, 1975.
Item 2 of this letter states weekly smear testing (wipes) will be performed when radioactive material is in use.
Contrary to this requirement, as of the dates of this inspection, weekly smear testing had not been performed during the semesters when radioactive materials were in use.

Paragraph 10 CFR 20.401(b), Records of Survevs, Radiation Monitoring, and Disposal, Title 10 of Code of Federal Regulations, Part 20, states, in part, each licensee shall maiatain records showing the results of surveys required by 10 CFR 20.201(b) to verify that waste disposals are in compliance with 10 CFR 20.301.
Contrary to this requirement, as of the dates of this inspection, survey records =ade on waste disposed of to normal trash had not been maintained to verify compliance with 10 CFR 20.301.

License Condition No. 13.A.(1) of Byproduct Material License No.
14-10271-02, states, in part, each sealed source designated for the purpose of emitting alpha particles shall be tested for leakage and/or contamination at intervals not to exceed three months.
Contrary to this requirement, on the dates of this inspection, leak testing of the Pu-Be sealed source and Howitzer had been performed during February, 1977; November, 1977; June, 1978; and May, 1979; intervals in excess of three months.
