Part 1 of the quality assurance program refers to an " attached organization chart," but no chart is attached. Please supply a chart showing the organiza. ion responsible for the program.

Part 1 of the quality assurance program assigas responsibilitier to
" Management and Authorized Personnel," and parts 4 and 5 assign respon-sibilities to " Authorized personnel."
Identify the responsible personnel (by job title) on the organization chart. That is, make the text and the chart compatible.
