This refers to your application dated December 29, 1978 requesting approval of your Quality Assurance (01) program as meeting the QA program requirements of 10 CFR 71.51.
In connection with our review, wc need the iniorciation identified in the enclosure to this letter. Please submit seven co7ies of your response to the enclosed request for additional inf' nation ithin 30 days following receipt of this letter. Also, please delete 6a through 6f and Enclosure 2 from the application.

Request for Additional Information 1.
Show the Radiographic Safety Officer (by title) on the organization chart (Enclosure 1).
2.
Iaentify who (by title) is responsible to insure that all QA functions are conducted in accordance with the latest applicable changes to documents.
3.
In part 6, provide a statement that written safety procedures concerning the receipt, handling, storage and shipping of packages for certain special form radioactive niucerial will be followed; that shipments will not be made unless all tests, certifications, acceptances, and final inspections have been completed; and work instructions will be provided for handling, storage, and shipping operations. Also identify who (by title) performs the critical handlino, storage, and shipping operation.

In part 7, provide a statement that nonconforming parts or packages will be handled in accordince with documented procedures; that status will be indicated by tag, lat el, marking, or log entry; and indicate who (by title) perfor.ns the regulato.y required inspections and tests.

In part 6, provide a statement that the records which follow will be identified and maintained in accordance with written procedures:
package approvals, procurement, inspections, tests, operating logs, audit results, personnel training and qualifications, shipment records, equipment description, and procedures.

The areas to be covered by audit are too limited.
Provide a statement that each activity will be audited at least once each year by an auditor having no responsibility in the activity being audited.
The acdited activities should include the activities of the Radiographic Safety Officer, training activities, record keeping, etc.
