Request for Additional Information 1.

Provide a statement that QA Program revisions will be made according to written procedures with management approval.

2.

Provide a statement that the QA Program will ensure that QC procedures, engineering procedures, and specific provisions of the package design approval are satisfied.

3.

Item 5.2 states:

" Shipping containers shall be... certified for use to us by the Nuclear Regulatcry Commission."

Replace this statement with a etatement that the Radiation Safety Officer shall assure that all radio-active material shipping packages are designed and manufactured under a QA Program approved by the Nuclear Regulatory Commission for all packages designed or fabricated af ter implementation e? the QA Program.

')rovide a statement that all documents related to a specific shipping package will be controlled through the use of written procedures and that all document changes will be performed according to written procedures approved by management.

5.

Indiccte who is responsible to insure that all QA functions are conducted in accordance with the latest applicable documents.

6.

Provide a statement that radioactive material will not be shipped unless al'. tests, certifications, acceptances, and final inspections have been completed.

7.

Provide a statement that inspection, test, and operating status of packages for radioactive material will be indicated by tag, label, marking, or log entry and controlled by written procedures.

8.

Provide a statement that procedures will be written and followed to control nonconforming parts or packages.

9.

In part 6.1, provide statements that a) the records will be maintained in accordance with a written procedure.

b) the records will be identified and retrievable.

c) a list of the records (and their specific locacion) will be maititained by the Radiation Safety Officer.

10.

Provide a statement that audit reporte will be evaluated and deficient areas will be corrected.
