In connection with our review, we do not agree with your assertion that the Senior Radiographer has no direct responsibility in the areas of radiography packages. We note that part 4.3 of the application lists his responsibilities for radiography packages. While it is acceptable to us that the Senior Radio-grapher perform audits, we require that each activity (including the activities of the Senior Radiographer listed in part 4.3 of the application) be audited at least once per year by an individual or team that has no responsibility in the activity being audited. We also note that the application of May 4,1979 is incomplete in that it lacks organization chart (s).
Position titles and reporting relationships are all we require on the chart (s), but individual names may be included if you wish.
Please submit seven copies of your response to the above within 30 days following receipt of this letter.
