Scope:

Gas Rentals /Lcwell Gas as licensee for the possession and use of by-product material for radiographic purposes recognizes its responsibility to the nuclear Regulatory Co-ission, the Cocconwealth of Massachusetts, its custcners and employees to establish policy and procedures for the safe use of radiographic devices.

Part of our policy and procedures under our license shall include this Qua.ity Assurance Progran. Responsibility for t'.is progran sFall be Gas Rental's/Lowell Gas, as raced on our license. Design and fabrication shall not be conducted under our license.

The Radiation Safety Officer is responsible for administration of this prog m ; as well as,

+m ining, certification, document control, and auditing.

The Radiographer (s) are responsible for WMing, storing, shipping, inspection, test, operations, and record keeping.

A copy of this Qmlity Assurance Progran shall be carried in the radiographic inspection vehicle. Additio-al copies will be cade available to supervisors and others requiring knowledge of same. A copy will remin in the docu=ents' control file.

The organization, authority, and responsibility for Radiation Safety and Quality Assurance exists cs follows:

The Radiation San ty Cc -4ttee sF1 (1)establishpolicy, regulations, and procedures, (2) review qualifications and training of individual radio-graphers, (3) review exi.,-dng procedures and facilities, and (!;) request license amendments, provide for training and inspect radiographic operations and facilities.

Fadiation Safety Officer (RS0)

The RSO is responsible for ca =ying out the recocnendations of the Radiation Safety Co=mittee, serving as liason officer with the liRC and state on license matters, proedg and disposing of licensed by-product caterial nna na,inistering the personnel conitorire prognm. He also insures that a trai"4"; prograa is provided, leak testing and instnt:ent calibrations perfored, sou ces returned for replacement by the manufacturer, records properly kept and assistance provided in emergency or accident situations.

He will conduct inspections of radiogmphic operations and file appropriate rerorts in accordance with the requirements of parts 19, 20, 21, 31, s_. and 4

license conditions. He shall also be responsible for na + istration of the Quality Assurance Program. He shall report directly to the Vice President of Opentions regarding Quality Assurance catters.

Radiognpher - Radiography Supervisor:

The radiographer has overall responsibility for actual on the job operations. He shall coordinate with 'he RSO on all catters pertni 'ra to safety, trainirs, and procedures. He m_111 insure that all radiation detection and monitoring devices are opemble, calibrated and maintained in accordance with applicable instruction:

requirements of these procedures.

He shall also be responsible to see that only the individuals listed on the license and authorized by the P2diation Safety Co-ittee use the radiographic source. He shall also be respons1ble for adhering to the Q'Mity Assurance Progm as described.

Quality Assumnce Prom The nanagement of Lowell Gas through the Vice President of Operations and the Radiation Safety Co-ittee establishes and implements this Quality Assurance Program. Training, prior to engagemen', for all Quality Assurance functions is required according to written proct:. ces. Quality Assurance Progran revisions will be =ade accorM g to written procedures and approved by the Eadiation Safety Co-4ttee. The Quality Assurance Prog c.m will insur-that all defined Q.C. procedures, engineering procedures, n-M specific provisions of the package design approval are satisfied. The progran will emphasize control of.the chameteristics of the package which are critical to safety.

These written procedures sh11 be provided by the Quality A~ surance I4"nger (in this case ES0) and kept on file for review and inspection.

The ESO shall assure that mdioactive material shipping packages are designed and -,"ni'actured under a Quniity Assurance Progran approved by the IEC for all packages designed or fabricated after the effective date of the Quality Assurance P_m gram. This *,71. be met by obtaining a certificate of compliance from the n:mufacturer.

I)ocument Control:

A I)ocument Control File will be established for the maintenance of all records relating to radiographic operations and in particular Quality Assumnce Procedures and =aterials receipt and use functicns.

ESO shall i.sure that all Quality Ass r ance functions a e conducted in accordance with applicable procedures and approved changes. This file shall be available for review by authorized audit persennel and IEC i.spection. 307 294 13T3

F2ndling, Storage, and Shipping:

Priortothepurchase, handling,stomge,and/ ors'11ptentofradioactive material, written safety procedures shall be followed. No receipt or shipment shall take place unless all tests, certificates of ccupliance, inspections, etc. have been completed. Instructions will be provided for b Aling, storage, and shipping operations. Only persons authorised to handle radioactive material under our license shall do so.

Inspection, Test, and Operating Status:

Procedures shall be followed for inspection, test, a.d operation of the licensed radiographic projectors supplied by Technical Operationa and approved for use 'mder our license. Appropriate tags and/or m,*ngs (labels),

as well as, appropriate log entry enH be =aintained and recorded in accordance with written procedures.

The RSO, as responsible agert, shall insure that mdiograpPJ personnel perfom inspections and tests in tecordance vihh written procedures.

Quality Assurance Records:

Recordsofpackageapprovals(withdetail),procurecent, inspection, tests, logs, audit results, training qualifications, and records of all chipments shall be m 4ntained. Equipment description and storage locations chall be nnintained, as well as, written procedures.

All appropriate records and procedures shall be nn'ntained. Records shall be identifiable and retrievable from the files. Lists of same shall be mintained by the Eadiation Safety Officer.

Audits:

Audits shall be carried out on a periodic basis at least once per year.

Mesters of the audit tean !'1 have no direct responsibility for the activity being audited. Results will be re.orded and reported to operations ca. age-ment, as well as, the Eadiation Sm :ty Cc=mittee. Written check lists will be maintained for operations and Quality Assurance Audits.

Findings shall be evaluated and deficiencies corrected. The results will be m intained on file.

Audits shall serve to evaluate the effectiveness of the Quality Assumnce hegren and recccmend changes as necessary. Consultants and others nct involved in the rndiographic operations 227 be utilised to recc=cend changes that would i= prove the safety of operations. Far2genent
