I APPENDIX A NOTICE OF VIOLATION The Rutland Hospital Docket No.:
30-07587 Rutland, Vermont 05701 Licensee No.:
44-14121-01 As a result cf the inspection conducted on March 26, 1981, and in accord-ance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980),
the following violations were identified.
Condition 14 of your license requires that licensed material be possessed and used in accordance with the statements, representations, and procedures contained in your letter dated May 6,1977, Exhibit 5 of which is your
" General Safety Instructions to be Followed by Laboratory Personnel."

Item 1 of these " Instructions" requires that liquid proof gloves be worn at all times while handling radioactive materials.
Contrary to this requirement, on the day of the inspection, March 26, 1931, personnel failed to wear liquid proof gloves while injecting radiopharmaceuticals.

Item 2 of these " Instructions" requires that personnel monitor hands and clothing for contamination before leaving the Department.
Contrary to this requirement, on the day of the inspection, March 26, 1981, personnel failed to monitor their hands and clothing for contamination before leaving the Department.

Item 3 of these " Instructions" requires that lead syringe shields be used for transfer and injection of all radioactive materials.
Contrary to this requirement, on the day of the inspection, March 26, 1931, personnel preparing radiopharmaceuticals failed to use syringe shields for the preparation of patient doses.
4.
Item 3 of these " Instructions" requires that holders be used for transport of injectable radioactive materials to other parts of the hospital.
Contrary to this requirement, on the day of the inspection, March 26, 1981, a radiopharmaceutical dose was transported to a patient's room without using the required holder.
5.
Item 6 of these " Instructions" requires that all nuclear medicine personnel wear monitoring devices while on duty.
These include both film badge monitors as well as ring monitors for those who are actually handling radioactive materials.

Centrary to this requirement, on th2 day of the inspection, March 26, 1981, one person working in your nuclear medicine department was not wearing the required ring monitor.
These are Severity Level IV Violations.

Pursuant to the provisions of 10 CFR 2.201, the Rutland Hospital is hereby required to submit to this office within twenty-five days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Under the authority of Section 182 of the Atomic Enargy act of 1954, as amended, this response shall be submitted under oath or affirma+ on.
