INSTRUCTIONS FOR COMPLETING NRC FORM 527, REQUEST FOR INFORMATION RELATED TO CONTRACTOR CHARGES -- IN ACCORDANCE WITH 10 CFR §170.51 Please read instructions carefully.

A licensee may request additional information related to the contract costs associated with their NRC invoice (in accordance with Part 170 of Title 10 of the Code of Federal Regulations)

1. Complete all items on page 1 (except section for NRC Staff only) of NRC Form 527 as follows:

Enter the licensee's name and address exactly as they appear on the invoice.

Enter the invoice, docket, EPID, and CAC numbers exactly as they appear on the invoice.

Enter the Pay Period End Date(s) exactly as they appear on the invoice.

Enter the company's business telephone number.

Enter the contact person's name and e-mail address.

(Note: Submission of this form does not change the due date of the subject NRC invoice).
