APPENDIX A NOTICE OF VIOLATION Tennessee Va' ley Authority License Nos. CPPR-162 CPPR-163 Phipps Bend, Units 1 and 2 Besed on the NRC inspection May 8-11, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items ha e been categorized as described in correspondence to you dated December 31, 1974.
A.
As required by Criterion V of Appendix B to 10 CFR 50 and as imple-mented by the PSAR Section 17.1A.5, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings and shall be accomplished in accordance with these instruc-tions, procedures or drawings". Procedure S0P QC-18 requires that test sieves used in analysis of concrete aggregates be calibrated every six months.
Procedure C-208 and specification CFB 300-01 require that concrete test cylinders be stored at 73.4 + 3 degrees Fahrenheit and at a minimum relative humidity of 95 percent until the moment of testing.
Contrary to the above, the following examples of failure to follow procedures were identified. Test sieve number 100 identified as CN/SI-19 was not calibrated on the required recalibration date of March 16, 1979 and measurements taken in the curing room between May 8-11, 1979 showed temperature anges of 69 to 81 degrees and relative humidity ranges of 91 t-36 percent.
This is an infraction for units 1 and 2.
B.
As required by Criterion V of Appendix B to 10 CFR 50 and as imple-mented by the PSAR Section 17.1A.5, "Activitics affecting quality shall be prescribed by documented instructions, procedures, or drawings and shall be accomplished in accordance with these instruc-tions procedures or drawings". Procedure R1SPM C-301 requires that reinforcing steel be stored off the ground on cribbiag to allow air circulation and to avoid trapping water.
Contrary to the above, several bundles of reinforcing steel in the northeast area of the Unit 1 powerhouse block were stored on the ground, or on cribbing with ends dragging on or buried in the ground.
This is an infraction for units 1 and 2.
