JUN 0119 APPENDIX A NOTICE OF VIOLATION The Chesapeake Corporation of Virginia License No. 45-08475-01 Based on the NRC inspection May 24, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
A.
As required by item 9 on the license, licensed material may only be used in a source holder of designated model number.
Contrary to the above, the cesium 137 sealed source of 50 millicuries used in a density gauge at the #17 Mud Tank was identified by label as *)eing in a Model S-SH-1A holder.
A source holder of this model num.ar is not designated on the license.
This 2s an infraction.
B.
As required, in part, by 10 CFR 19.11(a) and (b), " Posting of Notice to Workers", each licensee shall post current copies of 10 CFR 19 and 10 CFR 20, the license, and related documents and procedures referenced in the license.
If posting of the above documents is not practicable, a notice may be posted which describes the documents and states where they may be examined.
Contrary to the above, neither the documents described in 10 CFR 19.11(a) nor the notice described in 10 CFR 19.11(b) was posted.
This is a deficiency.
