This letter is transmitted in fulfillment of license condition 4.B of Amendment No.
4 to Operating License No.
DPR-74 which requires that the Indiana 6 Michigan Power Company provide for NRC staff review the results of qualification testing per-formed in conformance with IEEE 323-1971 requirements for all electrical terminations in all safety-related circuits inside containment for postulated steamline break and/or 1'oss of coolant accident conditions.

1 to this letter further amplifies information presented in our April 21, 1978 letter to you and was discussed with members of your staff on April 28, 1978.
This attachment includes the following reports:

Instrumentation Cable/Penetration Wire Interface in Cook Nuclear Plant Unit 2 Safety-Related Circuits Inside the Containment.
Control Terminal Blocks Inside Containment.

These reports include a specific description of Samuel Moore Cable.
Information concerning justi~"'cation of a 60 x 106 rad integrated dose in the pipe tunnel was also requested by your staff.
A final report on this item is currently under preparation and will be transmitted to you shortly.
