The inspection involved a total of 40
onsite inspector-hours by two NRC inspectors.
Results: Of the six areas inspected, one item of noncompliance was
identified in one area (infraction - improper storage of pipe sleeves -
Section I, Paragraph 3).

Licensee Action on Previous Inspection Findings
(Closed) L'nresolved Item (440/77-04; 441/77-04):
Placement of heavy-
weight concrete.
Previous RIII Reports (440/77-04; 441/77-04) and
(440/77-U6; 441/77-06) questioned specific aspects of the use of
iron oxide mortar in the biological shield wall.
Subsequent corre-
spondence between the licensee and RIII 7 arrowed tne areas in
question down to whether the weight and thermal conductivitiy of the
heavyweight concrete had been taken into consideration in the plant
design analyses. During this inspection, the inspector reviewed,
and discussed with the licensee, supplemental engineering work si.eets
which showed that GAI had incorporated the heavyweight concrete design
parameters into the appropriate seismic, dead weight, and thermal
analyses.
This item is considered to be resolved.
(Closed) Noncompliance (440/78-02-14; 441/78-01-14):
Failitre to docu-
ment and identify voids on concrete placement.
The previous RIII
report (440/78-02; 441/78-01) noted that National Engineering failed

to docuo.ent voids on a concrete placement in the Intermediate Building
on a Nonconformance Report (NR), or to identify the specific void areas
with a nonconformance tag or other means.
During this inspection,
the inspector inspected National Engineering concrete placement areas
in the Intermediate Building, and found no examples of unacceptable
volds which were not identified.
The inspector also selected one
specific area with voids at the 620 ft. level of the Intermediate
Building and reviewed the applicable Nonconformance Report, NR 447,
dated February 28, 1979.
NR 447 showed that the void area had been
properly identified and placed on hold pending chipout and further
evaluation. This item ir closed.

1.
Review of Licensee Actions on Previous Inspection Findings
The RIII inspector reviewed the licensee's actions relative to
the resolution of soecific inspection findings which were identi-
fied in previous RIII inspection reports, and which were still
in an open status prior to this inspection.
The items reviewed,
and the licensee actions relative to the resolution of the items
are discussed in the foregoing section of this report.
2.
Review of Licensee Actions Relative to IE Bulletins and Circulars
The Region III inspector reviewed the licensee's handling of
IE Bulletins and Circulars to determint whether an effective
system has been set up for evaluating and responding to the
Il documents, and whether the system has been effectively
implemented.
Included in the review were discussions with CEI
CQS and Licensing personnel, and review of the following documents:
a.
CEI Nuclear Design Instruction 35-0603, Revision 0, effective
January 18, 1979, " Coordination of Responses to NRC IE
Bulletins and Circulars."
b.
The CEI IE Bulletin Log, which contains copies of all bulletins
sent to CEI, and related correspondence, from Bulletins 77-01
.
through 79-03A.
c.
The CEI IE Circular Log, which contains copies of all circulars
sent to CEI, and related correspondence, fron Circulars
76-01 through 79-07.
The review indicated that, although Instruction 35-0603 was not
effective until January 18, 1979, it served primarily as a written
formalization of the practices wh3ch had been followed by CEI
previously in the evaluation of and response to bulletins and
circulars.
The inspector reviewed the documentation for all of the bulletins
and circulars logged by CEI to determine whether distribution was
made to the appropriate persons, whether the required evaluations

were performed, whether timely tesponses were sent to NRC, ar.i
whether the responses addressed the appropriate technical
questions.
The inspectors then c;1ected six specific bulletins
and one circular, which were considered to be of particular
significance, and performed a more detailed review of the evalu
ations and responses for those specific items.
The results
of the detailed reviews are discussed in Section II of this re' ort.
Of the 22 IF Bulletins issued during 1977 and 1978, ten bul1< tins
wer e either not applicable to the Perry Project or did not require
an answer. One bulletin (78-12), which has not yet been answered,
has received extensions in response time (per 78-12a and 78-12b)
to May 29, 1979.
The responses for ti:e remaining 11 bulletins
-ere considered to be adequate. With regard to IE Circulars,
a total of forty were issued during 1976, 1977 and 1978, with an
additional five issued in 1979 through March 20, 1979
Of the
45, 12 were not applicable to Ferry, and only four of the others,
all in 1976, required responses. The licensee'n re ords indicate
adequate consideration of IE Circulars.
Based on the review, the RIII inspector concluded that:
a.
CEI has implemented an effective system for the handling
of IE Bulletins and Circulars;
b.
The evaluation of and response to bulletins and circulars
has been perf ormed in a systematic and timely manner;
c.
For those bulletins selected and reviewed in more detail,
the technical evaluation performed by the licensee appears
to be adequate (see Section II).
All IE Bulletins issued during 1977 and 1978, except for 78-12
(inclo?ing 78-12a and 78-12b), are considered to have been adequately
evalutted snd responded to by CEI; all IE Circulars issued during
1976, 1977, 1978, and through 79-05 in 1979 are considered to
have teceived adequate consideration by CEI althcugh, as noted
is. Set'. ton II of this report, additional RIII inspections will be
made wiC regard to Circular 78-08.
3.
Ongoing Cens.ruction Activities
The inspector observed ongoing work activities and equipment
status in the Units 1 and 2 containment buildings, including
the Unit I reactor pedestal area, the Units 1 and 2 auxiliary
buildings, the intermediate building, the control complex, and
the fuel handling building.

On April 20, 1979, at azimuth 210 vu the Containment No. I weir
mat level, the inspector observed ten 36-inch diameter pipe
sleeves for the bioshield which were improperly stored, in that
the sleeves were not on dunnage, and there were ladders, chunks
of steel, and angle iron lying on the sleeves.
The inspector
reviewed the applicable Newport News specification, SP-96-4549-00
dated December 18, 1975, and found that Section 5:16 " Receiving,
Storage, and Handling," requires storage of the sleeves to be
in accordance with ANSI N45.2.2 Level D.
The inspector notified
the licensee that the storage of the sleeves, which does not
comply with the requirements of ANSI N45.2.2 Level D,
is also
in noncompliance with Criterion XIII of 10 CFR 50, Appendix B.

Except for the item cited above, ne items of noncompliance were
identified.

1.
Licensee Action on IE Bulletins and Circular - Un_its 1 and 2
The inspectoi reviewed the licensee's responses to IE Bulletins,
a.
as stated in Section I of this report.
b.
The inspector reviewed IE Bulletin Nos. 77-05,77-05A,
77-07, 7P-02 and 78-04.
Subsequent to the issue of these
five bulletins, IE Circular ho. 78-08, which includes all
of the information contained in these bulletins, was issued.
For tracking purposes, the bulletins are closed and future
inspection results covering these matters will be docu-
mented under Circular No. 78-08.
Open items described in IE Circular No. '8-08 are as follows
(440/79-04-02; %
'74-04-02).
(1) For connectors, the reviews of environmental prototype
test results are not complete;
(2)
For electrical penetrations, the licensee has not
accepted the final environmental and seismic test
results;
(3) For terminal blocks, the licensee has not accepted
test report Pen-TR-79-03 " Qualification Test Report
on Three Terminal Blocks for use in BWR Nuclear Power
Plants;"
(4) With regard to limit switches, the liceisee stated
that all Class IE switches located in the primary
containment will be seismically and environmentally
qualified for their application.
Supporting docu-
mentation will be made available for review.
The inspector also reviewed IE Bulletin No. 78-06.
The

licensee had performed a systematic review ef the Perry
design and equipment specification and confirmed that tio
Cutler-Hammer Type M, DC Relays, Catalog No. D23 MD were
being installed in Class IE equipment,

Review of Electrical Contractor's Training Records
The inspector reviewed four QC inspectors training records to
determine if they were certified in accordance with E-C QA
Procedure 4.1.4, Qualification and Certification of QC Personnel
and ANSI N45.2.6.
The records reviewed indicated the QC inspectors
were properly qualified.
No items of noncompliance were identified.
4.
, Electrical Components - Observation of Work Activities
The inspector observed partial and completed work on the electrical
embedded duct banks for the Emergency Diesel Generator Building.

banks were installed in accordance with drawing D426-630,
Revision B and Concrete Placement, E-C Procedure, 4.3.14, dated
April 11, 1979.
They were identified as DR-DG-FOST IB1, 2B1, 3B1,
4B1, SBl and 6B1 for documentation purposes. Nonconformance
Report (NR) CQE 45, dated April 9, 1979 was written against
2B1 and SBl due to interference with toe fuel oil pipe lines
running beneath the duct runs.
The inspector reviewed E-C docu-
mentation packages for the above mentioned duct banks.
No items of noncompliance were identified.
5.
Othcr Areas Inspected
a.
The inspector toured the plant site; including the electrical
contractor's cable reel yard, laydown area and the in-place
storage of switchgear components and found these areas in
accordance with procedures and specifications
b.
The inspector reviewed CEI's site procedures
- he direction
of Fire Brigade and Cutting and Welding activities,
c.
During this inspection, the inspector also reviewed several
American Nuclear Insurers inspection reports of fire
protection safety discrepancies and recommended corrective
actions.
CEI was correcting the discrepancies identified
by the American Nuclear Tnsurers.
No items of noncompliances were identified.
Exit Interview
The inspectors met with licensee representatives (denoted under Persons
Contacted) at the conclusion of the inspection on April 20, 1979.
The
inspectors summarized the purpose and findings of the inspection.
The
licensee acknowledged the findings reported herein.
