_ NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on March 26-30, 1979, it appeared that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:
A.
Criterion V of Appendix B to 10 CFR 50 states in part, " Activities affecting quality shall be prescribed by documented instructions d shall be accomplished in accordance with these instructions Energy Products Group (EPG) Procedure HS-GTAW-25, Procedure Specifi-cation for, " Multiple Layer Hard Surfacing by the Gas Tungsten Arc Pro.ess", Paragraph 4.13, " requires gas flow of 30-35 CFH."
Contrary to the above, a welder was observed performing Multiple Layer Hard Surfacing under Procedure No. HS-GTAW-25 with a gas flow rate of 25 CFH.
Corrective and preventive measures were taken concerning this deviation prior to the conclusion of the inspection, therefme no further response to this item is necessary.

Criterion IX of Appendix B to 10 CFR 50 states in part, " Measures shall be established to assure that special processes, including welding,... are controlled and accomplished... using qualified procedures in accordance with applicable codes...."
1he above requirement is supplemented by the Quality Assurance Manual, Section 7 " Welding", paragraph 7.1,d; which states in part, " Manufacturing qualific3 the procedure in accordance with the ASME Code....
Qualification test results are documented on the (PQR) QW-483 form, as specified in ASME Section IX...."
Paragraph QW-216.2,(d);Section IX of the ASME Code states in part, "A chemical analysis shall be obtained from the test assembly....
The chemical analysis of the sample shall be within the range of analysis given in the WPS."
Paragraph QW-282.4;Section IX of the ASME Code states in part, "The WPS shall be set up as a new WPS, and shall be completely requalified when any of the following changes are made....
For submerged arc welding, a change in the nominal composition or type of flux used...."

Paragraph QW-201.2, of Section IX of the ASME code, states in part, "The specific facts including the base metal Type and Grade (or chemical analysis and mechanical properties) and the essential variables... used in qualifying the WPS shall be recorded in a form called Procedure Qualification Record (PQR).
This form shall also record the test results."
Paragraph IV. 1, Welding Procedure Specification (WPS) No. HS-SA6, Revision 6, dated August 10, 1978, Hard Surfacing by Submerged Arc Process"; specifies the flux to be Linde 80-12X65 Mesh. and paragraph VI, states in part, " Analysis of the deposited weld metal shall have the limits for the critical elements as follows:
n 2.0 percent maximum...."
Contrary to the above requirements, the PQR number HS-1G-1, Revision 4, dated August 10, 1978, supporting WPS number HS-SA6, Revision 6, dated August 10, 1978, does not identify whether the deposited viald metal was within the acceptance limits for Mn. identified in the WPS, nor does the PQR state whether the flux used in qualifying the procedure, was the same as the flux prescribed by the W?3.
