5/11/79 - 5/18/79: Unit 2 Control Rocm surveillance as IE Operations inspector on 0800-1600 (uay) shift.

R.B. Sump level measurement calculations
-- Comments on Observations /sct.ivities:
(1) Aux Operators were frequentiy having difficulty performing mathematical calculations for Aux Building Ventilation System ri: lease rates (uCi/cc., uCf /sec).
The inspector frequently had to point out errors in the calculations.
The errors were corrected when pointed out but it did not seem obvious that other licensee personnel (shift foremen, shift supervisdr, etc.)
were auditing or reviewing these calculations to an extent needed.

7 JUN 1979 (2) An unnecessary Aux Building entry had to be made by the licensee to verify earlier measurements of verticle height of the gage ; sed to perform RB sump level measurements.

The A DHR Pump was found to have a hot bearing during an operability check. After Westinghouse personnel raised the oil reservoir approximately one inch the pump was run satisfactorily. The oil reservoir was bolted to a stand. This leads to a question of future reliability of that pump with respect to possible future movement of the oil reservoir, b.

Primary and BOP System parameters in Unit 2 Cor, trol Room (2) Unit 2 FHB wall boring for ADHR penetrations (3) Operator Training on Aux Diesel Generators (4) Technical Working Group meetings (GPU/ Met-Ed/B&W/W management) on 5/24 and 5/25.

Approximately t"rteen recovery procedures (Emergency, Operating, Modi.. cation) in draft form and forwarded comments to NRR on site. as part of their input in the review process.

(1) Unit 2 Control Room, Unit 2 Turbina Building (2) Unit 2 Chemical Cleaning Building (3) West side M Unit 2 FHB - cxcavation area for ADHR penetration (4) Aux Diesel Generator Area in Yard
-- Comments on Observations / activities (1) Aux Diesel Generator fuel lines from the storage tanks to the Diesel Generator have several valve stem and threaded fitting leaks.

RCS Pressure instrumentation has been failing at a rapid rate. Several transmitters failed in the past week. As of this date the only nor.nally installed RCS pressare instrumentation left in an operable state includes B loop (Wide Range) and one RCP seal cavity pressure transmittors.
A Heise gaga and pressure transmitter provide additional data from the pressurizer sample line. The licensee is attempting to detemine additional locations :o install additional instrumentation. -
