1036 Revision 0 03/01/78 1.0 GENERAL l.1 Purpose The purpose of this procedure is to describe the metht.
contrcl of read out devices which become inoperable or is strongly suspected 2
of being inoperable such that they are marked, documented and controlled until repair is affected.
1.2 Scope This procedure applies principally to the control of out-of-service instruments and read out devices which are required by the Technical Specifications.
It applies to meters, gauges, amplifiers, recorders when they become inoperative or are displaying what appears to be _..
incorrect information.

2.0 RESPONSIBIL ITIES

2.1 Shift Supervisors, Shift Foremen and Control Room Operators 1i 1
The Shift Supervisor and/or Shift Foreman and/or Control Room.
Operators are responsible to assure out-of-service ins'cuments are

identified and logged out-of-service.
They are also resr]nsible to assure that Work Requests are submitted to affect repair and remc e....
the out-of-service stickers from the instrements and closidg out the out-of-service instrument log entries. -
2.2 Maintenance Foremen

Requests on the out-of-service instruments are reported clear to the Shift Supervisor or Shift Foreman or Control Room Operator when the Work Requests are signed off as complete.

1036 Revision 0 3.0 REQUIREMENTS 03/01/78 3.1 Out-of-Service Instrument Control 3.1.1 When instrumentation required by the technical specifications is out of service or out-of calibration sue.h that the associated control room parameter readout displayed on a meter, gauge, amplifier, recorder, etc. is incorrect, it shall be appropriately marked.
3.1.2 The marking of incorrect or out of calibation control room readout devices will be done with yellow sticker tags (or other suitable tag where the below informatica may be displayed when yellow tags are not available),

3.1.3 Yellow sticker tags will be marked with the words
'Ou t-of-Service and will indicate the time, date and initials of the j~~

Shif t Supervisor or Shift Foreman who authorized the yellow

3.1.4 In addition to the above, the attached " Instrumentation Out-of-Service log" form will be utilized to control and monitor

The Work ' Request issue date must be marked in the 005 t
sticker tag log.
The Work Request must be nerked clearly that g
the 00S sticker tag number should be included in the Work 3.1.6 The cognizant Maintenance Foreman must insure all such Work Requests with 005 sticker tag numbers on them, are reported clear the the SHIFT SUPERVISOR or'SH_IFT FOREY. N or CONTROL

1036 Revision 0 03/01/78 3.1.7 The Shif t Supervisor or Shif t Foreman or Control Room Operator on clearance from the Maintenance Foreman will remove the 005 sticker tag and make a clearance entry in the 00S sticker log.
3.1.8 The 005 sticker log and the associated 00S sticker tags installed -
on control room readout devices will be reviewed on a weekly (7 day) basis by the Shift Foreman.
3.2 Use of Instrument Out-of-Service Log 3.2.1 An Instrument Out-cf-Service log will be maintained to aid in the control of Out-of-Service instruments.
The log book will normally be retained in the Control Room of the respective units. An example of the log sheet is attached.

3.2.2 The log sheet shall be filleo out when an Out-of-Service sticker tag is applied noting the tag number, 00S time and

date, associated Work Request date, affected device, reason I
sticker applied, and the Shift Supervisors or Shift Foremans or Control Room Operators initials.
Upon removal of the Out-of-Service sticker tag the removal section of the log shall be completed noting the time, date and Shift Supervisors or Shift Foremans or Control Room Oper: tors initials.
