1685 Revision 5 09/19/77 THREE MILE ISLAND NUCLEAR STATION STATION HEALTH PHYSICS PROCEDURE 1685 PROCEDURE COMPLIN4CE 1.0 PURPOSE The purpose of this procedure is to insure proper radiological protection practices are being performed at all times.

2.0 DISCUSSION 2.1 To maintain a high standard of operation in radiation protection practices, it is necessary to monitor the radiological protection practices of all personnel employed at Three Mile Island Nuclear Station at all times.

2.2 Monitoring will be conducted by the Radiation Protection Supervisor / Foreman, Radiation Protection personnel or other assigned personnel.

2.3 Violations of radiological practices shall be reported to the Radiation Protection Supervisor.

2.4 All personnel will be instructed as to proper health physics procedures and will be expected to udhere to these practices.

1685 Revision 5 09/19/77 5.1.1 If an individual observes an action contrary to established Health Physics Procedures, he should confront the violator and suggest that a safer procedure be followed.

He will also notify the Radiation Protecticn Department concerning the violation.

5.1.2 When violations of Health Physics Procedures are observed by any Radiaticn Protection perscnnel, the violator will be quickly notified and informed of the correct p ocedure.

The Radiation Protection Supervisor / Foreman will be notified of the violation.

5.1.3 The Radiation Protecticn Suoervisor/Foraman will perform the following:

5.1.3.1 Notify the individual's supervisor or foreman of the violation and instruct individual to cease work to correct the violation if not corrected.

5.1.3.2 Forward Radiation Protecticn violation notice (Form 1685-1) with section one completed to Radiation Protection Supervisor for approval and filing in individual's file.

No copies will be distributed.

5.1.4 For violations which are " Repeat Offenses" or of a serious nature (at the discretion of the Radiation Protection Supervisor),

perform the following:

5.1.4.1 Complete section two of the Radiation Protection Violation Notice.

5.1.4.2 The Supervisor of Radiation Protection and Chemistry and Radiation Protection Supervisor will review the incident with the individual and his Supervisor and agree on corrective acti on.

5.1.4.3.1 Original - Supervisor of Radiation Protection and Chemistry /

Individual's file.

5.1.4.3.2 Individual.

5.1.4.3.3 Individual's Supervisor.

5.1.4.3.4 Radiation Protection Supervisor.

5.1.4.3.5 Unit Superintendent.

5.1.4.3.6 Quality Control Supervisor.

5.1.5 If the violation is a third offense or of a serious nature (at the discretion of the Radiation Protection Supervisor) disciplinary action will be recommended to the Unit Superintendent for tPI individual (s) in vie'atier, of Health Physics Procedures.

5.2 Record of violation of Health Physics Procedures.

5.2.1 Observed violations shall be documented on Form 1685-1.

5.2.2 Rar: stion Protection Violation Notices will be retained in each individual's file.
