NPDES Per=it 'To. PA 0C09920 Three Mile Island ITuclear Station (CE)
. Sectica II A.5 of the above referenced permit requires prc=pt notification of all bypasses of treatment systems at IMI.
Please be advised that 7,400 and 7,100 gallens of water eere discharged on May lh and 15, 1979 respectively, to the influent of the Mechaahn' Draft Cooling Tower Discharge (MDCT),
thereby, bypassing the Industrial 'a'aste Treatment Syste= (I'aTS).

In order to prevent any inadvertent dise'.arges resulting from the DC Unit 2 accident, numerous preerutionary provisions have been i=ple=ented.
Specifically, the breakers vt.ich activate various su=p pu=ps were caution tagged "open" so the pu=ps could not operate.
In this instance, the contents of the turbine surp was not pu= ped to the LTS (as vould cecur under nor" 1 cperating procedures) since e'l three pu=ps had their breakers open (inactivated). ?so cf the turbine surp pu=ps pu=p to the L'IS and the third pump, the e=?rgency deluge pump, pu=p to the :CCT influent. Ac the Pa0ER is aware (frc= cur letters of :Tove=ber 3,1978, and December 29,1977),
the operatien of the ?artine Ecc= su=p deluge pu=p prevents the su=p frc=
overflcving and causing pessible equipment damage.
"When the Tarbine Ecc= su=p reached a high level the operator inad nrtently activated the breaker for the emergency deluge pu:.p.
The cperator should have activated the other turbine sump p=ps whiah would have pu= ped the su=p contents to the L'IS.
As the other nor= ally operating su=p pu=ps were inactivated, the deltge pump pu= ped = cst of the su=p centents directly to the :'DCT influent. The precautionary procedure of leaving the breakers open was implemented to provide assuranca that the 10 CFR 20 radiological limits vere satisfied prior to diccharge. This procedure did not, however, address the :TFDES bypass provision.

It is also believed that no oil and grease was present in either discharge.
This is based en the fact that there was approdmately two feet of
- rater in the suq after each pu=p Jown and the inability of the approd -ately two inches of oil on the surface to reach the suction of the pu" p.
- ^n In order to prevent a similar occurrence frc= happening, the fouoving actions have been initiated:
1.
SD-P-5 breaker (which ac.ivates the deluge pu=p) has been red tagged open.
2.
The controlling procedura for r;=p discharges has been revised to clarify the point that SD-P-3 cannot be used for routine Turbine Enilding St=p pu= ping.
3.
A vaming sign has been placed on the SD-P-5 breaker to alert the operators of:
A) The NPDES restrictions en its use and,
- 3) The require =ent to have a Shift Supervisor / Shift Foreman's per=ission prior to closing.
