Organization The final responsibility for the Quality Assurance (QA) Program for Part 41 d

71 Require ents rests with Space Science Services. Inc. Design and Fab-

",n rication shall not be conducted under this QA Progran. The QA Progran is

implemented using the following organization:

Donald A. Geiger, P.E. Gen'1 Mgr. Radiation Protection Officer David A. Geiger, Mgr. Ala. Operations, Asst. Radiation Protection Officer James H. Geiger, M r. Fla. Operations, Asst. Radiation Protection Officer.

6 The Radiation Safety Officer is responsible for overall adminisi. ration of the program, training and certification, document control, and auditing.

The Radiographers are responsible for handling, storing, shipping, inspection, test and operating status and record keeping.

Quality Assurance Program The management of Space Science Services, Inc. establishes and implements this QA Program. Training, prior to engegement, for all QA functions is required according to written procedures. QA Program W sions vill be made according to written procedures with management appru-s1. The QA Program vill ensure that all defined QC procedures, engineer'.ng procedures, and specific provisions of the package design approval are satisfied.

The QA program vill emphasize control of the characteristics of the package which are critical to safety.

The Radiation Safety Officer shall assure that all radioactive material shipping packages are designed and nanufactured under a QA Program approved by Nuclear Regulatory Co= mission for all packages designed or fabricated after the effective date of the QA Progras. This requirement can be satisfied by receiving a certification to this effeo - cm canufacturer.

Docu=e t Control All documents related to a specific shipping package vill be centrolled through the use of vritten procedures. All document changes vill be performed according to written procedures approved by waragement.

':he Radiation Safety Officer shall insure that all QA functions are conducted in accordance with the latest applicable < heges to these documents.

Handling Storage and Shipping Written safety procedures concerning the handling, storsgo and shipping of packages for certain special form radioactive caterial vill be followed. Shipments will not be made unless all tests, certifications, 3

acceptances, and final inspections have been completed. Work instruc-tions vill be provided for handling, storage, and shipping operations.

Radiography personnel shall perfor:n the critical handling, storage and R.a shipping operations.

Inspection. Test red Operating Status Inspection, test and operating status of packages for certain special form radioactive r.aterial vill be indicated and controlled by written M

pmcedures. Status will be indicated by tag, label, marking or log entry. Status of nonconforming parts or packages vill be positively h,0 maintained by written procedures.

Radiography personnel shall perform the regu Latory required inspections and tests in accordance with written proceduces. The Radiation Safety gp Officer shall ensure that these functions are perfor=ed.

Quality Assurance Records Records of package approvals (including references and drawings), procure-ment, inspections, tests, operating logs. audit results, personnel train-ing and qualifications and records of shipments vill be maintained.

Descriptions of equi cent and written procedures vill also be maintained.

The records vill be identifiable and retrievable. A list of these records, with their storage locations, will be maintained by the Radiation Safety Officer.

Established schedules of audits of the QA Program vill be perfor=ed using vritten check lists. Results of audits vill be maintained and reported to mana6e=ent. Audit reports vill be evaluated and deficient areas corrected.

The audits vill be dependent on the safety significance of the activity being audited, b';.t ecch e.ctivity will be audited at least once per year.

Audit reports vill be maintained as part of the quality assurance records.

Members of the audit team shall have no responsibility in the activity being audited.
