o D R A F T' Scope This investigation examined (1) reactor operations from the beginning of the

event (0400 on March 28) until stable operation after the restart of reactor coolant pump LA (2000 on March 28), snd (2) radiological consequences from the beginning of the event unt11 C001 on March 31 when releases of radio-
~
active material to the environ =ent had been stabilized and radiological controls were established to minimize in-plant radiation hazards. The investi-gation included examination of licensee activities to assess the cause of the incident, couipment operation, procedures followed, personnel training, onsite radiation protection, radiological effluent releases, radiological environ-mental measurements, and emergency actions.
This investigation does not include (1) review of activities af ter the periods specified, (2) nonradiological gadioeet-ivit3 releases, (3) assessment of envircamermal impact, (4) assessmeet of nonlicensee actions, (5) involve-ment of the NRC except to the extent that NRC involvement impacted licensee actions.
