Contrary to the above, it was observed that unused lov hydrogen velding electrodes were left in an cpen container exposed to the atmosphere for an extended pericd. This was in violation of site directive MCP-2-2 which requires that unused lov hydrogen filler =aterial in opened containers be returned to

Corrr'tive Actiens An investigation was begun upon discovery cf the above referenced situation, and the investigation disclosed that a.
the situation occurred 62. spite adequate procedural controls, b.
the velder who was responsible for the electrodes si= ply failed to report when they became missing, and c.
the use of these electrodes did 'not affect quality, in that the electrodes vere used only to mount a te=porary holding bracket, the function of which was to hold another co=ponent in position while the co=penent was being erected.

To avoid violations of this type in the future, the following preventative actions have been taken:

B & W's velding supervisors have instructed, in detail, the crib attendants in velding electrode control procedure, b.
