Dear Mr. O'Reilly:

This refers to the AEC/D.'.0 inspectica cenducted en Julf 22-25, 1973, and the findings which appear to be in violatica of AEC require-cents as discussed in the July 25, 1973 Exit Conference and delineated in AEC letter dated Septenber 7,1973.

The attach ent centains cur respense to those findings, a dis-cus. on where applicabic, and a statement regarding implecantation of corre tive acticn.

There was no decurentary evidence that Makeup Sys:ce Pump; MU-P-la and 13, stored in place in the Auxiliary Suilding, had been periodically rotated and lubricated.

This is required by 35'J Field Specificaticn FS-II-1, UE5C Nuclear Construction Precedura NCP-1-2 " Installation of Nuclear Auxiliary Purps", and UESC QC Procedure QC-3-2 " Storage Control".

Discuscien:

Failure to pr:perly raintain safeguard equipment had previcusly been identified by AIC inspectors in March, 1973. As indicated at that tire, a Preventive Maintenance Progrc with check lists was to be implenented by the Engineer in August, 1973 as corrective sc:ica to the proble cf ineceplete equipment lists and =aintenance require:ents.

Corrective Action:

1.

A Preventive Maintenance Program with storage records for the subject pumps was placed in effect at the sita on September 5, 1973 and both pumps were rotated and lubricated on September 6, 1973.

NC?-1-2 has been revised to reflect the Preventive Maintenance Progran as being the governing document for maintenance during storage. Sl-

Discussion:

UEaC QC Procedure QC-17-2 states in part that iters which are tagged

" Hold" or "Rej ect" cay be released f or purposes of relccatica, rework, testing, evaluaticn, conditicnal installatica, or any operaticn which is deened necessary by canagement perscenel. The design centrol ceas-urcs assured by this site group are inadequate in that this group with unlitited release authority does not qualify as the organica:icn respens-ible for the original design.

Authorication to install or use any "Rej ect" cr " Hold" iters is at variance with per: ices of 1CCFR50.

This requires that docu:entary evidence that the caterial and equipment confor: to the procurement requirements shall be available at the site prior to installa:len or use of such naterial or equipcent.

Corrective Action:

' ESC QC Procedure QC-17-2 is currently under revisien to correct J

these findings.

The proposed revisien will require that equipment ite=s that are tagged

" Hold" or " Reject" may be ccnditionally released. The specific lixitaticas for this release will be identified on a qualificc release form. The required level of =anage en: and technical approval to irplement a quali-fied release will be specified in QC-17-2.

In the case of this purchase order, the QA Data Sheet was revised in fulfill:ent of this precedure en August 17, 1972 after the issuance of all change orders.

The Data Sheet was then approved by Quality Assurance as a valid state-cent of the documentation required.

In taking this approval, 3D.* Quality Assurcnce was subject to their procedure No. 19A-4A whic*.. requires in p2rt that Quality Assurance review each purchase order change. The Quality Assurance revicw of the docu:entatica package was then cade against this valid state:ent of required dccumentatien. In initialing the sheet alcng-side each ite of required decurentaticn, the reviewer attested that the documentation was in accord with the statement of required decurcntaticn.

This sign-off is indicated en the sheet as August 29, 1972.

Corrective Acticn:

It is considered that 35W's (SSSS Supplier) procedure for handling change
