T*riREE MILE ISLAND UNIT tl0. 2 (TMI-2) STE/Ji GEriERATOR EASEL.INE INSPECTION The OI&E Daily Report dated November 23, 1977 and a subsequent Preliminary Notice frcm OI&E identified an apparent probica with the T!4I-2 stea=

generators. You will recall that 1141-2 has not yet received an operat-ing license and has just coapleted hot functional tests. The ECT inspection of stea:a generater "A" at TMI-2 folicwing the hot function-al test revealtddthe following:

1.

Eight (8) tubes with greater than 40% tube wall reduction.

2.

Ten 910) tubes with about 20% tute wall reduction.

3.

400 tubes with " mechanical dents 1 - 3 mils in depth.

4.

One tube did not allow passage of the eddy current prette Since this is not yet an operating facility, we discussed this matter with it. Bosnak, Chief HEB/ DSS, and with R. Silver, the PH/DF!i. Our primany concern with this problem is not with T14I-2 per se but rather with the meaning these events have on our overall understanding and present licenseing positions both with regards to the C&W probis2s experienced at Oconee and the Westinghouse /CE denting problems.

On December 2,1977, Dr. B. D. L'aw of my staff had a phone converstrion with Region I inspector Glen Walton and learned that the applicant had planned to recove four (4) - each of the four types of degradation iden-tified above - tubes for further examination in the laboratory on Decem-ber 6,1977. Results (4 the laboratory examinations are expected by the end of December.

As of Decester 2,1977, the inspection of the S/G-B is about half way through and has revealed "a couple of SM" that did not allow passage of the inspection probe. The inspection of the B-Generator is expected to be complete by Decuter 16, 1977. Mr. Walten will infom Dr. Liaw as soon as the inspection is ceapleted.

Since THI-2 is publy the first plant that has ever perform.i a baseline inspection of the steam generator tubes,.<a can not conclude whether or not these findings are unique for this faddlity. Easedden infor=ation available to us so far, we do not believe that tne findings at THI-2 contradict cur thinting on steam generater probices. We be-lieve this might shed some light on the initiation cechanista of Cconee's problem.

We are in close contaca with Bau and Mr. Walton of Region I and will be kept infon::ed of further develo;raents.
