License No. CPPP.-66 APPENDIX A NOTICE OF VIOLATION Based on the results of an NRC inspection conducted on February 7-10, 1977, it appears that certain of your activities were not conducted in full compliance with conditions of your NRC Facility License No.
CPPR-66 as indicated below.
These items are infractions.
A.
10 CFR 50, Appendix B, Criterion VII states, in part, " Documentary evidence that material and equipment conform to the procurement requirements shall be available at the nuclear plant site prior to installation or use of such... "; and Chapter 17.1.7.4 of the FSAR states, in part, "All equipment and material delivered to the site.... is subject to receiving inspection.... Receiving r
inspection is performed in accordance with written checklists and the results documented."
Contrary to the above, on February 9,1977, it was observed that inspection checklists had not been prepared for receiving inspection of certain purchase orders placed under Specification 2555-999. As i
examples; Valve No. V83A/B and No. NS-V84A/B which had been converted from manual operation to air operation and electric moto. operation were received on site and released for construction although checklists for performance of receiving inspection had not been provided; evidence of the seismic qualification of these valves was not provided despite the fact that they are classified as Seismic Class I in the Engineer's Valve List and in Specification 2555-103 in " Station Butterfly Valves."
Prior to the conclusion of the inspection, information was received which indicated that the identified valves are considered to be seismically qualified.

10 CFR 50, Appendix B, Criterion V states, in part, "Activitics affecting quality shall be presribed by documented instructions, procedures... and shail be accomplished in accordance with these instructions. procedures...."
Welding procedure 9WP-100N/C-6, paragraph 8.3 states, " Prior to welding, the surfaces to be welded shall be cleaned to bright clean metal. The cavity shall be examined by the magnetic particle or liquid penetrant method to assure complete removal of defects."
Contrary to the above, on February 10, 1977, it was observed, on welds identified as 2-MS-55, 2-MS-92, 2-MS-94, 2-MS-940 and 2-MS-800 that no magnetic particle or liquid penetrant examination was performed on the cavities prior to rewelding.
