License No. DPR-73 APPENDIX A NOTICE OF VIOLATION Based nn the results of the NRC inspection conducted on May 10-12, 1978, it appears that certain of your activities were not conducted in full ccmpliance with conditions of your NRC Facility License No. DPR-73 as indicated below.

These items are infractions.

A.

Technical Specification 4.6.1.1.a states in part, that, " Primary CONTAINMENT INTEGRITY shall be demonstrated:

a.

At least once per 31 days by verifying that:

All equipment hatches are closed and sealed."

Contrary to the above, the licensee did not verify that the equip-ment hatch was closed and sealed from February 9,1978 to May 12, 1978.

B.

Technical Specification 4.6.1.3.a states, that, "Each containment air lock shall be demonstrated OPERABLE:

a.

  • After each opening, except when the air inck is being used for multiple entries, then at least once per 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />, by verifying < 0.01 La seal leakage by pressure decay when the volume between the door seals is pressurized to 3,10 psig, for at least 15 minutes."

Contrary to the above, when the air lock was being used for multiple entries the seal leakage was verified < 0.01 La by pressure decay on March 19, April 7 and April 22,197E with the volume not pressur-ized to 3.10 psig and using a test interval of less than 15 minutes.

A pressure decay test from 10 psig to 0 psig with an acceptance criteria of at least ten seconds was used.

C.

Technical Specification 6.8.1 states in part, that, " Written pro-cedures shall be established, implemented and maintained..."

1.

Surveillance Procedure (SP) 2311-5, " Containment Integrity",

Rev. 1, was issued on February 24, 1978 and Temporary Change Notice (TCN) 2-78-292 to this procedure was issued on March 11, 1978.

2.

AP 1001 " Document Control", Rev.13, dated March 30, 1978, step 3.8.5.5, Surveillance Procedures, states in paragraph (b) that, "The documentation spec!fied must be filled in accurately and completely by the individual performing the surveillance..."

SP 2311-5, Rev. 1 with TCN 2-78-292, Appendix "B", step 3 states in part, that, "The air lock used for entry was leak rate checked by pressurizing both atmospheric door inner seals area...and verifying pressure did not decrease to O psig in <_

10 seconds." The step then has four labelled blanks for the four air lock doors with the caption " Time to Decrease to 0 psig."

Contrary to the above:

1.

SP 2311-5 wc.s performed on March 13, March 18, March 22, March 26 and April 13, 1978 without utilizing TCN 2-78-232; and, 2.

SP 2311-5 was performed on March 29, April 10, April 16 and April 18 without recording.the Time to Decrease to O psig for any of the four air lock doors.

D.

Technical Specification 6.11 states in part, that, " Procedures for personnel radiation protection shall bc prepared...ma.'ntained and adhered to for all operations involving personnel radiation exposure.

Health Physics Procedure 1612, " Monitoring for Personnel Contain-ment", Rev. 5, dated September 22, 1977, step 5.3.2 states, that, "All personnel leaving a Controlled Area shall monitor themselves and all hand-carried items before entering a clean area."

Contrary to the above at approximately 12:15 p.m. on May ll,1978, an individual left the Unit 2 Reactor Building Controlled Area via the equipment hatch door with an air bottle and hoses and entered the clean area outside without monitoring himself or his hand-carried items.
