Ai_sarent Infraction A 10CFR50, Appendix B, Criterion VII states, in part, "...These ceasures shall include provisions, as appropriate, for source evaluation and selection, objective evidence of quality furnished by the contractor or subcentractor, inspection at the contractor or subcontractor source, and examination of products upon delivery..."
Contrary to the above, flow restricting orifice plates for use in the =akeup and purification, core flood and decay heat syste=s received at the site on or about February 5, 1976 vere not subject to receiving irspection as required by site QC Procedure QC-2-2.
Corrective Action Taken The flow restricting orifice plates have been subjected to receiving inspec-tion cnd the results are available for your review at the TMI site.
Steps Taken To Prevent Recurrence Those Purchase Orders which are not vritten against a Burns at. Roe Specifica-tien and fall under QA/QC scope have been identified by the GPU Service Corporation Project office and have been brought to the attention of the Site Quality Assurance Manager.
In the future, all Jersey Central Power & Light Company Purchase Orders and Cnange Notices (for TMI-2) vill be reviewed by the GPUSC Project office and a detemination vill be =ade as to whether the QA/QC pregra= is applicable to the=.
The results of these reviews vill be forwarded to the GPUSC Site QA Manager for appropriate action.
Date That Ccnnliance Was Achieved Full co::fpliance was achieved on July 13, 1976 when all outstanding JCP&L Purchase Orders issued without a B&R Specification and which required site QA/QC coverage vere acted on by the GPUSC QA Manager.

penarent Infraction B 10CFR50, Appendix B, Criterien XVI states, in part, " Measures shall be established to assure that conditions adverse to quality, such as failures, malfunctions, deficiencies, deviations, defective =aterial and equip =ent, and ncnconformance are pro ptly identified and corrected..."
Contrary to the above, failure to cover electrical cable in outdoor storage for weather protection as required by Procedure ECP-3-2, which had been identified on October 23, 1975 was not corrected as of June 11, 1976.
Corrective Acticn Taken The following actions have been taken to correct this situatien:
1.
Required coverings have been applied to all reels in the south storage yard.
This was cc=pleted June 15, 1976.
2.
A thorough inspection was conducted en June 30, 1976 by the UELC Quality Centrol Department of all cable reel yards.
Inspection Eeport E-152h3 vhich docu=ents this inspection reports all cable reels were covered as per ECP-3-2.
3 On July 12,1976, DR #0614 was issued to identify reels which were 1
uncovered at the ti=e of the June 7-11 ins,meetion by NRC personnel.
DR #0614 vas dispositioned by issuing instructions to have representa-tive reels tested for insulation acceptability.
4.
Cable reels identified on ER #061h vill not be used in safety related circuits until test results have concluded that quality has not been degraded because of exposure.
Stecs Taken To Prevent Recurrence Cognizant ecnstruction supervisory personnel vill assure that covering of reels in storage yards are maintained as per ECP-3-2.
UE&C Quality Control Department vill include the south storage yard in the =cnthly surveillance report.
Date That Full Co=rlianca Was Achieved Full co=pliance was achieved on June 30, 1976 when Inspection Report E-15243 confir:ed that reels had been covered as per ECP-3-2.
DR #C61h written on July 12, 1976 further assures that cable reels identified as previously uncovered vill not be used in safety related circuits until cable adequacy is deter =ined by test.
