SE? 7 3 73 ENCLOSURE DESCRIPTION OF VIOLATIONS Metropolitan Edison Company Docket No. 50-320 License No. CPPR-66 Certain activities under your license appear to be in violation of AEC requirements. These apparent violations are considered to be of Category 11 severity.
1.
Criterion V, Appendix B,10 CFR 50 states in part, " Activities affecting giality shall be. prescribed by documented instructions, procedures, or drawings...and shall be accomplished in accordance with these instructions, procedures, or drawings."
Contrary to the above, and to site direc,tives:
The in-core penetration in the bottom head of the reactor a.
vessel was found to be open and unprotected.
This item was corrected prior to the conclusion of the inspection and need not be addressed in the reply.
b.
There was no documentary evidence that Makeup System Pumps HU-P-1A and 1B had been periodically rotated and Ir5ricated.
2.
Criterion VII, Appendix B,10 CFR 50 states in part " Documentary evidence that material and equipment conform to the procurement requirements shall be available at the nuclear powerplant site prior to installation...."
Contrary to the above:
Site directive QC 17-2, Revision 5, dated April 4, 1973, a.
permits " conditional installation" without defining the conditions under which it is permitted.
b.
A total of 13 Change Notices had been issued on Purchase Order 21851-LP, but the Quality Assurance Data Sheet for MU-P-1A did not establish that the pu=p met the requirements specified by the Change Notices.
