l CONDITIONS 1.

The licenses shall obtain all information from the foreign exporter necessc,ry to complete the shipper's porti'on of Form NRC-741, " Nuclear Material Tran actions Report."

If the licensee is the receiver of the material, he shall record both the shipper's and receiver's information on the Fom NRC-741 and transmit pursuant' to 'Section 70.54 of 10 CFR Part 70.

If the licensee is not the receiver of the material, he shall provide the ship,'er's informtion to the receiver of the material for inclusion on the receiver's Form NRC-741.

The licensee shall notify the appropriate regional Safeguards Branch, Office of Inspection and Enforcement, immediately, by telephone or telegram, if entry of any import shipment authorized by this license is refused by the U.S. Customs Service.

3.

Each time material is imported, the licensee shall show the original of the license to U.S. Customs as authority for the import.

The original of the license showing the imported quantity shall be surren-

'dered to U.S. Customs when the final transaction under the license is completed.

Except for the final transaction, a copy of the license showing the imported quantity shall be surrendered to U.S. Customf for each shipment.

All currendered document: shall be marked for immediate transmittal to the Office of International Programs, U.S. Nuclear l8 Regulatory Commission, Washington, D.C., 20555.
