OF KEY EVENTS Week Ending July 27, 1979 Pilgrim On July 27, the switchyard at Unit 1 was hit by lichtSe reackn1 tina in 1[h ina re of offsite power to the station. As a consequence, t or scramme. Un, startup attempt, incomplete valve closure was indicated. The plant was brought to cold shutdown to repair or replace the topworks of the valves. Startup is expected tomorrcw. Ci On July 26, GE advised us of a Part 21 notification relative to the environmental qualification of teflon coated motor leads to Limitorque operators. GE is now notifying plants of required corrective actions and schedules. Fort Calhoun Omaha Public Power District has acolied to the NRC for authorization to increase the power level of Unit 1 from 1429 MWt to 1500 MWt. Oconee On J'uly 24, while at 25% power, Unit I was shut down due to a steam generator tube leak. The shutdown is required by the Technical Specifications. Early Site Review Based on discussions >with Middle South Services, Inc., NRR expects to receive an early site review' application in May 1980 for a two unit nuclear plant in southern Louisiana. Denial of Petition for Rulemaking On July 18, the EDO signed a Federal Register notice of denial of petition for rulemaking. The petition requested the Commission to form a review bcard made up of responsible members of industry to review all pertinent regulations and to clarify the positions of the industry prior to issuance of all new regulations. Procosed Hansen Project On July 23, NMSS received an application to process uranium mill tailings that will be produced at the prcposed Hansen Project. Although Colcrado is an Agreement State, the Uranium Mill Tailings Radiation Control Act of 1978 re-quires that a byproduct license to possess tailings be obtained from NRC. Kerr-McGee Nuclear Corocration Acolication NMSS received an acclication and an environmental report from Kerr-McGee Nuclear Ccrporation for a source material license to recover uranium utilizing ion exchange techniques on minewater frcm multiple sites in Wyoming. Strike at Nuclear Fuel Services. Erwin, Tennessee The strike at the NFS facility has been resolved. Workers are to work en August S. 797 192 State Agreements On July 25, SP and CA briefed Senator Packwood's staff and the Senate Committee on Commerce, Science and Transportation regarding the forthcoming shipments of TMI waste to the Hanford low-level waste burial site. Nevada Gcaernor List re:--'ed the Beatty low-level waste burial site on July 24. NASAP/INFCE MPA circulated for comment a draft of the first semi-annual report to Congress on the status of domestic and international evaluations of alternative nuclear fuel cycle systems. FY 1980 Budaet The House and Senate Appropriations Conference Committees met and approved an appropriation for the NRC totalling $363,340,000 and 3,034 permanent positions. OFFICE OF NUCLEAR REACTOR lEGULATION WEEKLY ITEMS OF INTEREST (Week Ending July 27,1979) Pilarim Unit No. 1 ~ The switchyard at Pilgrim station was hit by' lightening at 1:00 a.m. today (Friday, 7/27/79) resulting in loss of offsite pcwer to the station. As a consequence, the reactor scrammed and the plant was isolated by MSIV cl os ure. The diesels started immediately and SRV's opened automatically. HPCI started manually in test mode. RCIC was started manually and used to control water level. Minimum water level reached during the transient was about 8 feet above top of active core, or about 1 1/2 feet above ECCS automatic start level. Offsite power was restored and diesels were secured at about 3:00 a.m. With SRV's relieving to the torus, pressure went down to about 600 pounds ' before startup was attempted. During the bicwcown, no particular effort was made to control pressure, but only to maintain water level in the vessel. On startup attempt, "D" SRV cycled between open and close, and "A" SRV showed high tail pipe temperature, indicating incomplete valve closure. The plant was then brought to cold shutdown to repair or replace the topworks of the SRV's. They expect to startup tomorrow. A PN is being prepared for issuance this morning. OFFICE OF STANDARDS DEVELOPMENT IMPORTANT EVENTS FOR THE WEEK ENDING JULY 27, 1979 1. Meeting of the Fredrickson Comittee on Biological Effects of Radiation: On July 24, 1979 the Interagency Counittee on Bio!69ical Effects of Radiation, chaired by Dr. Donald Fredrickson, NIH, met in Buiesda. F. Swanberg, RES, and R. Purple and M. Parsont, SD attended. Dr. A. Upton, NCI, presented a discussion of the current and proposed folicwup studies of the offsite TMI population. He presented also the report of the Subcomittee which he chairs (deliberations of which have been reported in previous weekly events). The Committee accepted the Subcommittee report with minor corrections. Dr. Michael Parsont, SD, presented infor: nation on NRC licensee data reporting requirements and some TMI dose data. In addition he discussed the NRC/ EPA Feasibility Planning Study contract as it pertains to Three Mile Island wor kers. Dr. Upton was asked to prepare an additional repcrt on the TMI worker population radiation exposure and the records thereof. This report is to include all of the informatinn provided by NRC to NIOSH on TMI data records as requested by Secretary Califano. DOE and 000 representatives gave an overview of their programs to synthesize dose data resulting from fallout weapons testing. A CDC representative spoke on their efforts to get affected states (in Hestern U.S.) to set up a regional council to deal with health effects studies related to weapons testing. The Committee agreed that this was a good idea. A draft protocol was circulated that would establish the Comittee's procesares for reviewing all Federally-funded epidemiologic studies. All Comittee members were asked to furnish coments. A draft letter, from the Secretary of HEW to appropriate House Overs;ght Comittees, reporting on the status of HEW's efforts to coordinate Federal research on the biological effects of ionizing radiation was also circulated. Committee members were asked to coment on this draft within a week. [M. Parsant - 443-5854] 2. Cn July 18,1979, the E00 signed a Federal Register notice of denial of petition for rulemaking, PRM 7-1, that will be published in the near future. The petition requested the Cognission to form a review board made up of responsible members of industry to review all pertinent regulations and to clarify the positions of the industry prior to issuance of all new regulations. By letter dated July 18, the EDO informed Walter P. Peeples, Jr., the petitioner, that PRM 7-1 was denied on the grounds that the NRC policy and procedures for direct distribution of proposed and effective amendments of NRC regulations to 797 204 ENCLCSURE C licensees and other interested persons: (1) Fully satisfy the objectives in the petition; and (2) Assure that affected licensees and applicants are aware of all proposed and effective regulations of a substantive nature, without the need to form a review board as requested in the petition. OFFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest Week Ending July 27, 1979 Cvorus Mines Corocration - Proposed Hansen Project On July 23, Cyprus Mines Corporation sumitted to the Uranium Recovery Licensing Branch an application to process uranium mill tailings (byproduct material as defined in the Uranium Mill Tailings Radiation Control Act (UMTRCA) of 1978) that will be produced at the proposed Hansen Project. The project will be a conventional, acid leach mill with a 4,500 ton per day capacity in Fremont County, Colorado. Although Colorado is an Agreement State, UMTRCA requires that a byproduct license be obtained from NRC to possess tailings. Meetino with DOE and States on Inactive Mill Site Remedial Action A member of the Uranium Recovery Licensing Branch participated in a DOE sponsored workshop in Denver, Colorado, on July 20 on the DOE remedial action program at 22 inactive mill tailings sites. The program authorized by the Uranium Mill Tailings Radiation Control Act (UMTRCA) of 1978 calls for States to participate in cleanup of sites and contribute a portion of cleanup costs. NRC has an oversight role in these actions. Receipt of Acolication from Kerr-McGee Nuclear Corcoration_ The Uranium Recovery Licensing Branch received an application and an environ-mental report from Kerr-McGee Nuclear Corporation for a source material license to recover uranium utilizing ion exchange techniques on minewater from multiple sites within the South Powder River Basin, Wyoming. The mine-water is currently pumped from the mine workings, treated, and released to the Iceal drainage pursuant to NPDES permits without recovery of uranium values. Incident Resconse Planning A draft of the NMSS Emergency Preparedness Action Plan has been developed and distributeG to the staff for comment. The plan is now being integrated with the earlier report of the FC Incident Response Planning Group for incorporation into the Task Force's report to the Comissioners, scheduled for August 3,1979. Items of Interest 2 Uccrade Rule Guidance Comoendium First drafts of all the effectiveness test questionnaires not included in the published Upgrade Rule Guidance Compendium have been completed. Drafts of all the information request sheets of the Standard Fonnat and Content Guide have also been completed. These drafts will be distributed for coments. Value/Imoact Analysis - MC&A Upgrade Rule The remaining areas for which value/ impact analyses will be performed to support the MC&A Upgrade Rule have been identified, scoped, and forwarded to Battelle-Pacific Northwest Laboratory for technical task analysis initiation. Scent Fuel Protection In Transit We are continuing to respond to a series of letters prompted by the new rule for the protection of spent fuel snipments in transit and related acti vi ti es. Among the inquiries we are responding to this week is one from Senator Helms of North Carolina and one from Delegate Grayson of the Virginia House of Delegates. We are also respending to remarks made by Mr. Sheldon Mayer of Tri-State before the U.S. Senate Commerce Committee. Scent Fuel Route Surveys During the period between July 24-29, Safeguards Division teams surveyed the route proposed by Duke Power Company for transporting spent fuel frcm Oconee, S. C. to McGuire, N.C. and the route proposed by the University of Missouri from Columbia, Mo., to the Savanah River plant. Workshoo on Behavioral Science and Physical Security NMSS representatives attended the Fourth Behavioral Science Workshop spcnsored by the Defense Nuclear Agency on July 25 and 26,1979. The program included a presentation by the Lawrence Livermore Labs of an NRC spcnsored study entitled, "Use of Analogous Incidents in Characterized Safeguards Threats", and an open meeting of the interagency Physical Security Evaluation Advisory Group to address behavioral science research needs. Strike at Nuclear Fuel Services, Emin, Tennessee The strike at the Nuclear Fuel Services facility located in Erain, Tennessee has been resolved. PNO-79-270 and -270A Rancho Seco - Results of Analysis and Testing on Class 1 Piping Supports - As a result of analysis and testing of piping supports as required by IE Bulletin 79-02, the licensee discovered piping supports with safety factors lower than required. The olant was taken to hot shutdown and pipe supports were upgraded. (Closed) b. PN0-79-271 Indian Point Unit 2 - Contamination of Workers - Six employees were externally contaminated. The employees were decontaminated and given whole body counts. (Closed) c. PN0-79-272 Marble Hill Units 1 & 2 - Recurring Concrete Placement Deficiencies - On July 20, 1979, the Director of Region III (Chicago) and members of his staff met with members of Public Service of Indiana corporate management to discuss the concrete placement activities at the Marble Hill nuclear site. The licensee has new stopped further Class I concrete work pending evaluation of the contractor's corrective actions. (Closed) d. PNO-79-273 Hatch Units 1 & 2; Farley Units 1 & 2 - Southern Company Task Force Recommendations - A task force formed by the Southern Company issued its report containing 20 recommendations designed to improve the safety of nuclear power plants operated by the Genrgia Power Co. and the Alabama Power Co. (Closed) e. PNO-79-274 Transportation Incident Involving Medical Isotopes - A delivery truck containing medical isotopes collided with an oncoming vehicle, injuring both drivers. Although contamination was initially suspected, it was subsequently deterinined that all packages were accounted for and were free of contamination. (Closed) f. DN0-79-275 Mallinckrodt, Inc., St. Louis, MO - Lost Shipment of Licensed Byproduct Material - A package of radioactive material being shipped from St. Louis to Charlotte, NC did not reach its destination. The package contained 63 millicuries of iodine 131, 100 millicuries of molybdenum 99, and less than 2 millicuries of iodine 125. The licensee has put a tracer on the shipment. 00T and FAA are assisting in the search. (Closed) 9 PNO-79-276 Rancho Seco - Washington Post Inquiry Regarcing IE Investigation of Rancho Seco Design Error Allegations - A reporter for the Washington Post informed IE that an NRC employee alleged that the NRC investigation relating to design errors at Rancho Seco was a whitewash. The reporter was given rebuttal information. (Closed) h. PNO-79-277 Oconee Unit 1 - Steam Generator Tube Leak - The reactor was shut down on July 24, 1979 due to an apparent steam generator tube leak in excess of Technical Specifications limits. PNO-79-278 Washington Nuclear Project No. 1 - Construction Fatality - An ironworker installing reinforcing steel in the steam generator wall fell 37 feet. The worker was taker, to a local hospital where he died from the injuries. (Closed)

j. PNO-79-279 Mallinckrodt, Inc., St. Louis, M0 - Lost Shipment of Licensed Byproduct Material - A package of radioactive material being shipped to Waterville, Maine did not reach its destination. The package contained 500 millicuries of molybdenum 99. The consignee has put a tracer on the shipment. D0T and FAA are assisting in the search.

PN0-79-280 Crystal River Unit 3 - Failure of Emergency Diesel Generator During Surveillance Testing - During surveillance testing of both diesel generators, a fire occurred in the exhaust manifold of one diesel gener-ator. The fire was self-extinguished. The licensee subsequently deter-mined that the fire was caused by an excessive fuel rich mixture on engine startup and aided by excessive oil and carbon accumulation in the manifold. Corrective measures were taken. (Closed) 1. PNO-79-281 Zion Unit 2 - Reactor Trip Due to the Loss of a Feedwater Pump Caused by Personnel Error - A reactor trip from 90% power occurred due to 1*ne loss of one of the two operating feedwater pumps caused by personnal error. An engineer inadvertently backed down the hand speed changer on an operating feedwater pump instead of a feedwater pump that was out of service thereby causing a reduction of feedwater flow and subsequent plant trip.,(Closed) m. PN0-79-282 Saint Peters General Hospital, New Brunswick, NJ - Film Badge Exposure of 10 Rems - A filn badge assigned to an x-ray technician showed a dose of 10 rems for the month of June. Film patterns indicate the badge was exposed in a stationary position or during a single exposure of short duration. The individual's work assignments did not include handling radioactive materials. The licensee is conducting an investigation to determine if the dose was rrceived by the individual. (Closed) n. PNO-79-283 Marble Hill Units 1 & 2 - Construction Deficiencies Noted by ASME Inspectors - An inspection team of the National Ecard of Boiler and Pressure Vessel Inspectors recommended that the ASME certification issued to Public Service of Indiana be suspended pending correction of a number of code violations. Region III (Chicago) has dispatched an investigation team to deternine the extent of code violations, the relationship of these to the QA/QC program, and whether that program is functioning properly. (Closed) o. PS0-79-284 Waterford Unit 3 - Plant Evacuation - Construction forces at Waterford were evacuated due to a chlorine gas release by a chemical plant nearby. There were no injuries to site personnel. (Closed) p. PNO-79-285 Schlumberge'r Technology Corp., Houston, Texas - Lost Sealed Sources - Five scaled sources were lost overboard in the Gulf of Mexico about six miles from Grand Island, Louisiana. The sources were in shielded containers within a tool box which was loaded on a pallet on a boat. PNS-79-78 River Bend Unit 1 - Bomb Threat - A bomb threat was received by a guard at the main gate. Plant management was notified and a search of principal buildings was conducted. No bomb was found. They found standing by Intervenors and admitted three contentions in this first steam generator replacement proceeding. Chu, Vice President of Te1 power and three members of his staff met with J. R. Shea, J. D. Lafleur, and M. Peterson of IP 9 discuss U.S. export policy and future nuclear programs in Taiwan. On Tuesday, a ten-member delegation of South Korea newspaper, radio, and television journalists ' met with staff members of IP, OLB/NRR, and PA to discuss (a) NRC's role in domestic and export licensing, and (b) the pro-bable implications of TMI-2 on such factors as safety, operator training upgrading, and accident response. On Tuesday, Messrs. G. Roukens and G. A. De Vrey of the Dutch Nuclear Office of Labor Inspectorate, Ministry of Social affairs met with staff members of DPM, DSS, and DSF. to discuss quality assurance, operator licensing, reactor systems safety, and accident analysis. On Thursday, Mr. Bo Rydnert of Lutab (a Swedish Consulting Group) and Consultant to the Swedish Government Ccmmittee on Nuclear Reactor Safety met with J. J. Holman of OLB/NRR and W. S. Farmer of RSR/NRR to discuss the TMI-2 control room layout and operator actions in relation to human engineering (man-machine interaction) aspects of the TMI-2 incident. Mr. Rydnert also met with C. OFFICE OF STATE PFOGRAMS ITEMS OF INTEREST WEEK ENDING JULY 27, 1979 State Acreements The Assistant Director for State Agreements program and a repre-sentative of Congressional Affairs briefed Senator Packwcod's staff (Oregon) and representatives of the Senate Ccmmittee on Commerce, Science and Transportation on July 26 regarding the meeting to be held with Washington officials on July 27 and the forthcoming shipments of TMI waste to the Hanford low-level waste burial site. Nevada Governor List reopened the Beatty low-level was te burial site on Tuesday, July 24. In reopening the site, he required by executive order that: (a) waste generators provide the S tate with a Compliance Certificate warranting that the shipment conf orms to both Federal and State requirements for shipment, transportation, and disposal, and (b) waste brokers who ship or transport commercial low-level radioactive waste into the State for disposal certify that' they have conducted an external visual and dose rate inspection of packages and that no items of non-compliance were found, and (c) waste carriers certify to the S tate that the vehicle is properly placarded and secured and'that all shipping papers as required by U.S. DCT have been properly; executed. FY 1980 Budget The House and Senate Apprcpriations Conference '.cmmittees met and approved an appropriation for the NRC totalling $363,340,000 and 3,034 permanent positions. This Apprcpriatiens Act is expected to be signed by the President in mid-August.
