Request for Additional Information Regarding Gases 101 Transfer of Control to ChemPro Technologies Inc.
Date: Monday, July 29, 2019 2:05:00 PM Good afternoon Mr. Summers, This is in response to the amendment request dated June 27, 2019, indicated a change of control from Gases 101, Inc. to ChemPro Technologies Inc.s for exempt distribution license number 42-35292-01E and device registration certificate NR-1417-D-101-E. In order to complete our review please address the following questions:
- 1. Please confirm that the label currently included in Attachment 3 of 3 of the registration certificate NR-1417-D-101-E, which includes a license number, will continue to be used on the external housing of the ChemPro.
- 2. In the letter dated February 28, 2019, Mr Jaakkola stated that The QA program for the ChemPro Technologies is the same as with the Gases101. Since control of the certificate is being transferred to ChemPro Technologies Inc., please provide an updated quality assurance program identifying ChemPro Technologies Inc. as the owner/executor of the QA program. Also, we noted that Gases 101, LLCs quality assurance program contains program elements related to maintenance and service. Our understanding is that the elements of maintenance and service will not be handled by ChemPro Technologies Inc.
- 3. Please provide a copy of the ChemPro Technologies Inc. radioactive materials possession license issued by the State of Texas.
As this is a request to transfer the registration certificate and exempt distribution license, the responses to the questions should be submitted by representatives from ChemPro Technologies Inc. that are authorized to make commitments on the companys behalf.
If you intend to submit the information to us via email, the information should be submitted in an official company letter as an attachment to an e-mail. Please submit the requested information within 30 calendar days of the date of this email. If we have not received the complete information within 30 days, we will consider your amendment request as having been abandoned by you. This is without prejudice to the submission of a complete new amendment request.
