99900100/79-02 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on October 2-4, 1979, it appears that one of your activities was not conducted in accordance with NRC requirements.

Criterion V of Appendix B to 10 CFR 50 states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

The deviation from these requirements is as follows:

Standard operating procedure 4.C, dated 1/16/75, with regard to variation reports for nonconforming material found during processing, states in part, "4.C.I.

The following process must be used in all cases to properly complete a variation report..

Be sure the following steps are completed before turning in the V.R. to be processed..

Operator - Person the variation is charged to signs here to show that he has seen the V.R.

Manufacturing Supervisor Approval - Supervisors in all departments where the rejection occurred must sign here."

Contrary to the above, the three (3) following variation reports were not signed by the Operator and/or Manufacturing Supervisor.

No. 49526, dated 9/21/79 for housing casting 2.

No. 49052, dated 8/4/79 for a clutch housing casting.

3.

No. 49580, (date not noted) for a cover casting.
