Appendix A NOTICE OF VIOLATION Perry Memori?1 Hospital License No. 12-15671-01 Based on the inspection conducted on November 28, 1979, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.

Items 1 and 2 are infractions.

1.

Material submitted with the application dated June 27, 1978 referenced in license condition 15 states linearity checks of the dose calibrator will be performed quarterly.

Contrary to the above, no linearity checks have been conducted on the dose calibrator from July 18, 1978 to November 28, 1979, a period in excess of one quarter.

2.

Material submitted with the application dated June 27, 1978 referenced in license condition 15 states the Medical Isotope Committee will meet quarterly.

Contrary to the above, the Medical Isotope Committee has not been meeting quarterly.
