This refers to the inspaction conducted by Messrs. D. G. Wiedeman, W. P. Reichhold and J. Pagliaro of this of fice on November 27, 28 and 29,1979, of activities authorized by NRC byproduct material License No. 13-15586-01 and to the dis-cussion of our findings with you, Mr. Stan Huber, Gary Lingenfelter, R.T.,
Warren Troyer, R.T. and Judy Gatz, R.T. at the conclusion of the inspection.
This inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The inspection consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.
During this inspection, certain of your activities appeared to be in noncompliance with NRC requirements, as described in the enclosed Appendix A.
This also refers to an enforcement meeting held at NRC, Region III office, 799 Roosevelt Road, Glen Ellyn, Illinois on Friday, December 7, 1979 at 2:30 p.m.
The purpose of this meeting was to discuss the corrective action planned or taken for each item of noncompliance and to discuss the enforcement options available to the NRC.
From the above referenced enforcement meeting, it is our understanding that the items of noncompliance outlined in the attached Appendix A are presently being corrected and you are in the process of terminating your private practice license and will apply for two individual institutional licenses.
If our understanding of your actions as described above is not in accordance with the actions being implemented, please contact this office immediately.
This notice is sent to you pursuant to the provisions of Section 2.201 of the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations. Section 2.201 requires you to submit to this office within twenty days of your receipt of this notice a written statement or explanation in reply, including for each item of noncompliance: (1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved.

Consequently, in your reply to this letter, please describe what action will be taken or planned, to improve the effectiveness of your management control over your present and future licensed activities, so as to preclude the re-currence of these or similar items of noncompliancet We will gladly discuss any questions you have concerning this inspection.
