APPENDIX A NOTICE OF VIOLATION Nondestructive Inspection Services, Inc.
License No. 47-11883-01 Based on the NRC inspection October 10 and 12, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indi-cated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
A.
As required by 10 CFR 20.101(b)(1), no licensee shall possess or use licensed material in such a manner as to cause any individual in a restricted area to receive in any period of one calendar quarter from radioactive material in his possession a dose to the whole body in excess of 3 rems.
Contrary to the above, a person in your employ received a dose to the whole body of 3,950 rems during the calendar quarter July 1, 1978 to September 30, 1978.
This is an infraction.
B.
As required by 10 CFR 20.405(a), each licensee shall make a report in writing within 30 days to the appropriate U.S. Nuclear Regulatory Commission Regional Office, with a copy to the Director of Inspection and Enforcement, U.S. Nuclear Regulatory Commission, Washington, D.C.
20555 of each exposure of an individual to radiation in excess of the applicable limits in 10 CFR 20.101.
Contrary to the above, as of October 10, 1979, the day of the inspection, an exposure of 3.95 rems occurring during the third quarter of 1978 was not reported to the U.S. Nuclear Regulatory Commission.
This is an infraction.
C.
As required by 10 CFR 34.21, radiographic exposure devices measuring less than four inches from the sealed source storage position to any exterior surface of the device shall have no radiation levels in excess of 50 milli-roentgens per hour at six inches from any exterior surface of the device.
Contrary to the above, on October 10, 1979, the day of the inspection, radiation levels of 2000 milliroentgens per hour were measured at six inches from an Automation Industries Model 520 radiographic exposure device located in your source storage room.
This is an infraction.

As required by 10 CFR 20.203, each high radiation area shall be conspicuously posted with a sign bearing the radiation caution symbol and the words
" Caution, High Radiation Area".
A high radiation area is defined as such levels that a major portion of the body could receive in any one hour a dose in excess of 100 millirem.
Contrary to the above, on the day of the inspection, October 10, 1979, radiation levels of 2000 millirem per hour were measured at six inches from a radiographic exposure device in your source storage room. These levels constitute a high radiation area; the source storage room was not posted with a " Caution, High Radiation Area" sign.
This is an infraction.
E.
As required by 10 CFR 34.43(c), a physical radiation survey shall be made to determine that each sealed source is in its shielded condition prior to securing the radiographic exposure device.
Contrary to the above, on October 10, 1979, the day of the inspection, it was determined that no radiation survey had been performed on a Automation Industrie 520 radiographic exposure device to assure that the source had been returned to its fully shielded condition, as evidenced by radiation levels of up to 6000 milliroentgens per hour measured by the inspector at the surface of the device.
This is an infraction.
