AUG 2 01979 APPENDIX A NOTICE OF VIOLATION Texas Pipe Eending Company of License No. 52-13632-01 Puerto Rico, Inc.
Ponce Radiographic Facility Based on the NRC inspection August 1, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
A.
As required, in part, by 10 CFR 34.33(b), pocket dosimeters shall be read and exposures recorded daily.
Contrary to the above, pocket dosimeter readings had not been recorded on the day of inspection, from the dosimeters used during radiographic operations conduct;d on July 30, 1979.
This is an infraction.
B.
As required, in part, by 10 CFR 19.11(a), each licensee shall post current copies of the operating procedures applicable to licensed activities, or (b) post a notice that describes the document and states where it may be examined.
Contrary to the above, the " Operating and Radiation Safety Instruc-tions for Industrial Radiography" procedures could not be located by the licensee's representatives.
This is an infraction.
C.
As required, in part, by 10 CFR 34.43(d), records must be maintained of surveys that were made to determine that each sealed source is in its shielded condition prior to securing the radiographic exposure device.
Contrary to the above, survey records were not maintained of surveys performed on July 30, 1979, when a radiographic exposure device was secured.
This is a deficiency.
