A.

The prime contract number is hereby changed from the number "NRC-09-79-349 " to the number "NRC-1.0-80-395."

Invoices for work performed under Phase II of this contract should reflect the new contract number specified above and accounting appropriation data set forth in bloc _k no.10 of this modification no. four (4).

- Within a period of ten (10) workdays after submission of task 2 and task 3 deliverables, the NRC will provide comments and/or approval of such deliverables.

P_roject Status Reports Under Phase II, project status reports are required and are due on the first day of each month. These reports shall include an updated Gantt Chart,'a description of activities performed the preceding month, a general project status update, problems encountered or anticipated, and personnel changes from the sixteenth day to the last day of the month. A status meeting will be held between the COAR and the Contractor after delivery of each monthly status report, On the sixteenth day of each month, the Contractor will provide an abbreviated status report citing problems encountered or. anticipated, and personnel-charges from the first to the fifteenth day of the month. Additional informal status meetings-between the 00AR and the Contractor may be held as needed."

Regarding Article II - Period of Performance, add the following sentence:

"The period of performance for work required under Phase II shall commence as of the effective date of this modification no. 4 and shall expire within forty-six (46) weeks after said effective date."

Article III - Consideration, Estimated Cost, Fixed Fee and Oblication is revised as follows:

1.

Paragraph 1 is revised to read as follows:

"1.

It is estimated that the total cost to the Governr.ent for full performance of this contract will be $464,437.00 of which the sum of $425,961.00 represents the estimated reimburseable costs, and of which $38,476.00 represents the fixed fee."

2.

Paragraph 3 is revised by deleting the amount "$122,439.00" and substituting in lieu thereof the amount "$.264,437.00."
