5 Atlas Industrial Manuf acturing Co.

Docket No. 99900301/79-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on September 10-14, 1979, it appears that certain of your activities were not conducted in accordance with NRC requirements.

Criterion V of Appendix B to 10 CFR 50 states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished." Deviations from these requirements are as follows:

The Atlas Industrial Manufacturing Company (Atlas), Quality Assurance Program, Revision 2, section 5.0 states in part:

"The activities af fecting quality are prescribed in instructions, drawings, procedures, and manufacturing standards. They provide for compliance with the Code, Design Specifications and Procure-ment Documents. They include criteria for determining accepta-bility of activities on a quantitative and qualitative basis.

Contrary to the above:

Visual inspection procedure #140 did not contain or reference the acceptance criteria of " Tolerance for Vessel Shells" (NB or NC-4221),

" Maximum Of fset of Aligned Sections" (NB or NC-4232), " Reinforcement of Welds" (NB or NC-4426). (See DetailsSection II, paragraph B.3.c.(1))

Instruction and/or procedures, were not available which addressed the ASME Code requirements for " Temporary Attachments and their Removal" (NB or NC-4231.2).

The Atlas Industrial Manufacturing Company (Atlas), Quality Assurance Pro-gram, Revision 2, section 10.1.2, states in part:

". Persons performing examinations and test shall insert the Serial No. of the instrument used in the remarks section of the traveler."

Contrary to the above, it was observed that the serial number of the instruments used to verify the dimensional acceptability of parts, had not been recorded on the two travelers reviewed for job number 3482.
