YELLOW CREEK NUCLEAR PLANT UNITS 1 AND 2 - REBAR BENDING NOT IN ACCORDANCE WITH SPECIFICATIONS - NCR YC-023 - SECOND INTERIM REPORT On August 3,1979, M. J. Gouge, NRC-0IE Region II, was informed that the subject nonconformance was determined to be reportable in accord-ance with 10 CFR 50.55(e). This was followed by our first interim report dated September 4, 1979. Enclosed is our second interim report.

We expect to cubmit our next report by February 8, 1980.

ENCLOSURE YELLOW CREEK NUCLEAR PLANT UNITS 1 AND 2 REBAR BENDING NOT IN ACCORDANCE WITH SPECIFICATION NCR YC-023 10 CFR 50.55(e)

EC,0ND INTERIM REPORT Description of Deficiency This nonconformance report identified a problem in which partially embedded rebar developed a crack during bending by plant construction employees. Review of the proposed corrective action and action to prevent recurrence on this nonconformance report revealed that Division of Construction procedures for bending partially embedded rebar did not implement the requirements of General Construction Specification G-2, Revision 2, January 9, 1978. Specifically, G-2 now requires joint approval of both Engineering Design (EN DES) and Construction employees before bending or straightening of partially embedded rebar is performed. Up to the present time, Construction has not been notify'ing EN DES before bending partially embedded rebar. This problem apparently exists at all TVA nuclear plant construction sites.

Interim Progress Construction procedures at all construction sites have been revised to require Construction to obtain approval from EN DES before bending partially embedded rebar. TVA is continuing the investigation of safety concerns of bending partially embedded rebar before EN DES approval.
