NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-10 Docket No. 50-237 Based on the inspection conducted on September 4-28, 1979, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below. These items are infractions.
1.
10 CFR 20.203(c)(2) requires that entrances to high radiation areas be maintained locked except during periods when access to the area is required.
Contrary to the above, on September 6,1979, the resident inspector found the cage door entrance to a posted high radiation aret on the northeast end of the Unit 2 turbine floor to le unlocked and unattended.
He further determined that the door had been unlocked 0755 hours0.00874 days <br />0.21 hours <br />0.00125 weeks <br />2.872775e-4 months <br /> on September 4, 1979.
This ie a repetitive item of noncompliance since the same problem was identified twice previously in NRC Inspection Reports No. 50-10/
78-31; No. 50-237/78-29; No. 50-249/78 9 and No. 50-10/79-03; No.
50-237/79-03; No. 50-249/79-03.

Unit 2 Technical Specifications, Section 6.2.A requires that detailed procedures involving operation of components involving nuclear safety of the facility be prepared, spproved, and adhered to.
Shift Operating Routines, Appendix C, requires that the high radiation area entrances to the turbine be verified locked on a daily basis.
Contrary to the above on September 5 and 6,1979, the licensee failed to verify that the northeast cage door high radiation area entrance to Unit 2 turbine was locked.
3.
Unit 1 Technical Specifications, Section 6.2.A.6 requires that detailed written procedures involving preventative and corrective maintenance operations which could have an affect on the safety of the facility be prepared, approved and adhered to.
Dresden Maintenance Procedure, DMP 4100-1, " Fire Protection Procedure for Use of Heat Sources in the Plant," requires that one or more individuals in each work area where heat sources are being used be designated to watch for potential fire.
Contrary to the above, on September 13, 1979, while making a routine tour of Unit 1 primary containment, the resident inspector observed welding in progress with no fire watch present.

Appendix A 4.
Unit 1 Technical specifications, Section 6.2.B requires that radiation control procedures shall be maintained, made available to all station personnel, and adhered to.
Radiation Control Standards Procedure 37-1-E-3, " Work in Controlled Areas (Radiation Areas and High Radi-ation Areas)," requires that personnel not eat, drink, smoke, or chew in those controlled areas.
Contrary to the above, on September 13, 1979, while making a routine tour of the Unit 1 primary containment (a posted radiation area),
the rasident inspector observed a contract construction person chewing a cigar.
