This refers to your application dated October 4,1979, requesting approval of your Quality Assurance (QA) program as meeting the QA program require-ments of 10 CFR 171.51.
In connection with our review, we need the information identified in the enclosure to this letter. Please submit seven copies of your response to the enclosed request for additional information within 30 days following receipt of this letter.

Provide an organization chart which includes, by position title, the Radiation Safety Officer and Radiography personnel.
2.
In part 2.1, include a statement that the management of the Niles Steel Tank Company has established and is implementing the QA program.
3.
In part 4.1, insert "been" after "have" in the second sentence.

In part 6.1 insert " procurement," after " approvals," in the first sentence.
5.
Your application of October 4, 1979, refers to a specific model shipping package and includes a USNRC Certificate of Compliance for that shipping package. Please eliminate the reference to the specific shipping pack-age and delete the Certificate of Compliance. The quality assurance program is applicable regardless of the shipping package which is used.
