Rupture disc failed on HPCI exhaust causing trip of HVCI.

Failure occurred during SI with unit on line at 98% power.

Conditions Prior to Occurrence:

Unit was on line at 98% power. SI 4.5.E.1.d was being run.

Unit 2 was at 100% power.

Unit 3 was in a refueling outage.

Action specified in the Technical Specificat:en Surveillt.nce Hequ:rements met

SI's were run on RHR, Core Spray, ADS, RCIC, and Diesel Generator.

Teflon sheet failed providing no protection to rupture disc under vacuum condition.

It is believed that a flaw or defect may have been present in the Teflon sheet.

There was no significant resulting chain of events and no danger to health or safety of the public.

Corrective Action

The rupture disc was replaced and a DCR was issued to purchase an improved type rupture disc.
