No items of nonconformance and no safety items were found.

Infor.arion about the concrete voids in the tendon galley is included in this report.

The repair procedure is in-ciudad as an attachment to the report.

This procedure has been reviewed by Region I and we believe that it is adequa:e oince no structural-integrity has been lost.

Our feeling is that protection of the exposed rebar is the primary necessity for re_: sir / Since DRL has expressed an interest in reviewing the repair procedure before the repairs are made, our in-spec:crs have told the licensee that we would defer comnents n'o MZO-ED until the review by Headquarters is completed.

I: :.s noted that MET-ED is holding up repairs in the areas Of the tendons to be used until we notify them that the procedure is considered to be adequate.

Repairs in the areas of the unused tendon conduits, however, will proceed.

nadaquate inspection of the concrete forms, prior to pouring, appears to be the cause of the voiding.

Subsequent to the voiding, the licensee tightened up on this inspection area.

?o further evidence of this deficiency has been detected.

7ae licensee believes that additional testing is not necessary to confirm that there are no more voids.

Reasons for this posizion are discussed in the report.

SCOPE A special announced inspection was made at the 2452 Mwt Pressurized Water Power Reactor under construction on Three Mile Island near Middletown, Pa.

The purpose was to inspect the concrete voids uncovered when the forms were removed from the ceiling of the tendon galley.

Mr. D. Whitesell, Reactor Inspector (Construction),

Region I, accompanied the assigned inspector and wrote portions of this report.
