Three Mile Island Nuclear Station Unit I Docket No. 50-289 Nuclear Service River Water (NSRW) Pump Failure

Dear Dr. Knuth:

You were notified on March 4, 1974, of a situation which may be consid-ered reportable under 10CFR50.55(e), " Conditions of Construction". This situation related to the failure of NSRW Pump "A".

DESCRIPTION OF INCIDENT During the Hot Functional Test Sequence, the subject pump experienced a mechanical failure. This failure consisted of pump shaft fracture in the pump-motor shaft coupling area. An alternate pump was activated and the Hot Functional Test Sequence resumed without further operating difficul-ties or apparent da= ages.

CORRECTIVE ACTION TAKEN The subject pump was dismantled for inspection and engineering review.

In addition to pump shaft fracture, it was found that the shaft coupling key was sheared and the motor support brackets cracked.

The motor and pump have been returned to their respective vendors for complete inspection and repair as required. Following their engineering review, changes will be incorporated as required. When the results of the manufacturer's evaluation are received, they will be available for your review of the Three Mile Island Site.
