Office of Inspection and Enforcement, Region I U.S. :luelear Regulatory Ccnmission 631 Park Avenue King of Prussia, PA 19h06

Dear Sir:

Three Mile Island IIuclear Station, Unit 1 (TMI-1)

Operating License :Io. DFB-50 Docket :To. 50-289 As indicated in our letter of July 7, 1977, regarding a radvaste shipment leak incident, we are submitting a follow c.p information report to ecmplement the infornation provided therewith.

Although final liner test results are not available, it is cur cpinion that the cause of the liner leaks was acidic attack of the mild steel container at

?ocations of incenplete coverage with the internal corrosion resistant coating.

To assure eccplete coating, each of the empty liners presently cn site ic in the process of being reccated. After reccating, each liner will be tested using an internal to external conductivity test in accordance with an apprcved proceiure. This reccating procedure and subsequent testing will be under 0,uality Assurance personnel surveillance.

Freliminary results have been received frce our consultant laboratory which indicate the presence of certain potential solidification inhibitors, e.g.,

cil and evaporatcr anti-fca= agents, in the radvaste streans. As a result, the radvaste tc solidification agent ratic has been ad, justed to assure optimum radwaste solidification.

~' hose filled liners currently en hand with kncvn defects will be packaged in reinforced voeden boxes with void space filled with an absorbent packing material. Tnese boxes will then be shipped la acccrdance with L9CFF.173.392 requirenents.

3.,H.

Grier, Director August 5, 1977 GQL 103c We have instituted Quality Assurance requirements and procedu es to insure that, through contract specifications and supplemeatary documentation, liners procu ed in the future have adequate internal coating with assurance of ecmplete coverage.

We consider the above present and future corrective actions more than sufficient to preclude any future recurrence of tl e incident of June 13, 1977 Sincerely,

/
