Organization The Quality Assurance Program shall be the respen-sibility of Ahex Corp., Research Center, Mahwan, N.J.

The Radiation Safety Of ficer, Anthony F, Regina Jr, will be responsible for overall adninistration, training, certification, document control, and auditing of this CA Program per 10CFR71 b.

2.

Cuality Assurance Procram a.

Radiographers will be train 2 to handle, store, ship, inspect, test, operate, and keep records of all packages for the transport of radicactive material.

b.

The Radiation Safety Officer will insure that all radicaraphers are properly instructed..er Appendix 3 and Secticn 111 (12) of Abex's " Operating Procedures isnd Regulaticns Isotype Radiography" dated Feb, 14,1975, License No. 29-01208-02.

c.

The oP.ly packages to be used for the transpor cf radioactive material will be defined by Section 9

( Authorized use) of li anse no. 20-01208-02 title page. In addition the medel 428 (T.O.) source chargar will be used for the transport of 150 curies or less of 60.

d.

The necessary operating instructions and safety precautions are decurented for each trInspertation device in Appendix 3 and will be part of the training p rog ram.

The Radiation Safety Officer will insure that all radioactive shipping packages are designed and man-ufactured under a Quality Assurance Program approved by the Nuclear Regulatory Commision (this pertains to packages designed or fabricated after / July 1978).

Manufactures must supply Abex a certification of complience to 10CFR71.

Changes related to specific shipping packages will be approved by the Radiation Safety officer and be incorporated into the QA program per this Section.

3.

Document Control a.

All documents related to a specific shipping

package will be centrolled through the use of established procedures.

b.

All document changes will be performed according to established procedures.

c.

Document changes will be written procedures approved by management.

d.

Latest document changes will be impleted by the Radiation Safety Officer.

4.

Handling, Storage, and Shipping a.

Handling, Storage, and Shipping of packages will be performed by tested and certified radiographic personnel, b.

All procedures per Appendix B of Lic. No. 29-01208-02 will be cc= plied with in addition to the following.

Shipments will not be made unless all tests, certifi-cations, acceptances, and final inspections have been complied with. If excessive leakage or any other discrepancy is detected the Radiation Safety v..

All shipping packages will be stored within Vaults A or B.

5.

Inspection, Test and Cperating Status a.

The Radiation Safety Officer will insure that the inspections and tests required for packages have been performed by trained Radiographers.

b.

Status will be indicated by tag, label, marking, or log entry.

c.

Non-conforming packages will be identified, isolated, and tagged as such. Technical Operations will be notified of discrepant packages.

6.

Quality Assurance Records a.

Records of this QA Program will include 1)

All test and inspection results of packages 2)

Audit Results 3)

Qualification of personnel 4)

Shipments and Storage Locations b.

The Radiation Safety Officer will be respcnsible for all records.

7.

Audits a.

The Radiation Safety Officer will conduct periodic audits of the Quality Assurance programs using a written checklist.

b.

All results will be reported to the Manager of the Experimental Foundry.

c.

Audit records will be evaluated and deficiene areas corrected.

Audits will be performed based on the safety and activity of the facility.

Audit reports will be maintained as part of e.

the quality centrol records and the responsibilit'/

of the Radiation Safety Officer.

At least one audit per year will be conducted by an audit team with no responsibility in the activity being audited.
