Maine Yankee Response to Reference (b)
In accordance with the request made in Reference (b) f or certain information and actions by Maine Yankee relative to the events described, the following information is provided.
Maine Yankes has completed a review of all events which have occurred since the start of plant operation. We have determined that events similar to those described in reference (b) have never occurred at this facility. With regard to the request that a determination be made as to whether similar occurrences could occur, and whether the potential exists for problems relative to such occurrences, Maine Yankee feels that based upon the information provided, comon mode failure did not. contribute to the events described. Maine Yankee recognizes that inuependent random failures will always remain a possibility, however, plant design and existing surveillance and quality assurance require-ments provide reasonable certainty that potential failures will be detected and corrected to provide high assurance of the reliability of safety related systems.
A review has been conducted as to the adequacy of existing management policies and procedures to ensure that events similar to those described in reference (b) would be vigorously pursued and analyzed to identify potential failure modes not previously considered, which could lead to a significant reduction in the ability of safety systems to function as required. Maine Yankee utilizes Plant Information Reports (PIR) and Licensee Event Reports (LER) as m echanisms to describe events of significance which could have the potential of signifying previously unconsidered failure modes. All PIR's and LER's are reviewed in detail and discussed by both the Plant Operations R eview Committee and the Nuclear Safety Audit and Review Comittee. Additionally, copies

Based on the above, Maine Yankee feels that current company policies and procedures provide adequate assurance that all significant plant occurrences receive a thorough r eview and evaluation.
Maine Yankee has long recognized the need to avoid challenges to the protective features of the, plant, and has incorporated into current surveillance procedures those cautions felt necessary to assure this objective.
A review has been performed of.:11 applicable surveillance procedures. This r eview has reinforced our opinicn that all appropriate cautions are currently included in these procedures.
